MYH9基因Rs4821480位点单核苷酸多态性与IgA肾病病理及预后关系研究

摘要 目的:探讨IgAN患者MYH9基因Rs4821480位点基因单核苷酸多态性与病理及预后关系。方法:经肾组织活检确诊148例IgAN,56例随访。外周血提取DNA,采用限制性片段长度多态性分析(PCR-RFLP)Rs4821480位点不同基因型与IgAN病理及预后的关系。结果:(1)三种基因型病理免疫荧光IgA、病理免疫荧光C3、镜下血尿、病理HASS分级、蛋白尿程度等无统计学差异。(2)Kaplan-Meier生存分析整体比较,从肾活检到肾功能减退时间,TT基因型快于其他基因型,基因型组两两比较,GG基因型与TT基因型有统计学差异。(3)采用Cox比例风险模型将多因素对预后的影响进行分析,结果显示发病时年龄、高血压、蛋白尿、CCR影响肾病患者预后。结论:Rs4821480位点基因型与病理无关,T等位基因的可能影响患者预后。 Objective:To investigate the association of single nucleotide polymorphism of MYH9gene Rs4821480locus with pathological and prognostic features in patients with IgAN.Methods:Renal biopsy was performed in148patients with IgAN as the study case,56cases were followed up.Extraction of DNA from peripheral blood,restriction fragment length polymorphism(PCR-RFLP)was used to analyze single nucleotide polymorphisms of Rs4821480locus in MYH9gene.The relationship between the different genotypes and the pathology and prognosis of IgA nephropathy was analyzed.Results:(1)No statistically significant difference was found between the three genotypes,pathology,immunofluorescence,IgA,pathology,immunofluorescence,C3,microscopic hematuria,pathological grading of HASS and proteinuria.(2)Kaplan-Meier survival analysis overall comparison,from renal biopsy to renal dysfunction,the TT genotype was faster than its genotype.Compared with genotype,the genotype of GG and genotype of TT were statistically different.The Cox proportional hazards model was used to analyze the effects of multiple factors on prognosis.The results showed that age,hypertension,proteinuria and CCR influenced the prognosis of patients with nephropathy.Conclusion:The genotype of the Rs4821480locus is independent of pathology,and the T allele may affect the prognosis of the patients.
