摘要
目的探讨舒经活血汤结合关节镜治疗胫骨平台骨折的临床效果。方法选取2016年3月～2018年2月因胫骨平台骨折在江西省永修县人民医院就诊且中医证型属于气滞血瘀型的90例患者纳入研究,依据随机数字表法分为对照组和观察组,每组各45例。对照组单纯采用关节镜辅助复位内固定术治疗,观察组则联合舒经活血汤口服治疗,12周为1个疗程,治疗结束后比较两组治疗前后D二聚体(D-D)、超敏C反应蛋白(hs-CRP)改善情况,比较两组的关节功能(HSS评分)情况及关节功能总优良率差异。结果治疗后观察组D-D、hs-CRP水平均低于对照组,差异有统计学意义(P<0.05);治疗后观察组HSS评分总分高于对照组(P<0.05);观察组关节功能总优良率为97.78%(44/45),高于对照组的80.00%(36/45),差异有统计学意义(P<0.05)。结论对胫骨平台骨折患者采用舒经活血汤结合关节镜治疗效果显著,可促进术后康复,改善关节功能,值得推广。
Objective To investigate the clinical effect of Shujing Huoxue Decoction combined with arthroscopy in the treatment of tibial plateau fractures.Methods From March 2016 to February 2018,90 patients with tibial plateau fracture in People′s Hospital of Yongxiu County,Jiangxi Province and TCM syndrome type of Qi stagnancy and blood stasis were included in the study and were divided into control group and observation group according to the random number table method,45 cases in each group.Patients of the control group were treated with arthroscopic assisted reduction and internal fixation only,while patients in the observation group were treated with combination of Shu jing Huoxue Decoction,12 weeks as a course of treatment.After the treatment,the improvement of D-D and hs-CRP before and after treatment were compared between the two groups.The joint function(HSS score)and the total excellent rate difference of joint function of the two groups were compared.Results After treatment,the levels of D-D and hs-CRP in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total score of HSS in the observation group was higher than that in the control group(P<0.05).The total excellent and good rate of joint function in the observation group was 97.78%(44/45),which was higher than that in the control group(80.00%)(36/45),and the difference was statistically significant(P<0.05).Conclusion The treatment of tibial plateau fracture with Shujing Huoxue Decoction combined with arthroscopy can improve the postoperative rehabilitation and improve the function of joint.It is worth popularizing.
