摘要
目的探讨肝切除配合纤维胆道镜治疗肝胆结石的临床效果。方法 80例肝胆结石患者,以简单随机法分为对照组和联合组,各40例,对照组采取肝切除取石术治疗,联合组采取肝切除联合纤维胆道镜治疗,比较两组患者临床效果、手术时间、术中出血量、住院时间并发症发生情况、残石及复发情况。结果联合组治疗总有效率为92.50%,高于对照组的75.00%,差异具有统计学意义(P<0.05)。两组手术时间、术中出血量比较差异无统计学意义(P>0.05);联合组住院时间短于对照组,差异具有统计学意义(P<0.05)。联合组并发症发生率、残石率、复发率分别为2.50%、7.50%、7.50%,均低于对照组的17.50%、25.00%、35.00%,差异具有统计学意义(P<0.05)。结论肝切除联合纤维胆道镜治疗肝胆结石效果显著,能有效降低术后并发症发生率和残石率,且手术安全性高,值得临床应用。
Objective To discuss the clinical effect of hepatectomy combined with fiber choledochoscopy in the treatment of hepatobiliary calculi.Methods A total of 80 hepatobiliary calculi patients were divided by simple random method into control group and combined group,with 40 cases in each group.The control group was treated with hepatectomy,and the combined group was treated with hepatectomy combined with fiber choledochoscopy.Comparison were made on clinical effect,operation time,intraoperative bleeding volume,length of stay incidence of complications,occurrence of residual stone and recurrence between the two groups.Results The combined group had higher total treatment effective rate as 92.50%than 75.00%in the control group,and the difference was statistically significant(P<0.05).Both groups had no statistically significant differences in operation time and intraoperative bleeding volume(P>0.05).The combined group had shorter length of stay than the control group,and the difference was statistically significant(P<0.05).The combined group had lower incidence of complications,residual stone rate and recurrence rate respectively as 2.50%,7.50%and 7.50%than 17.50%,25.00%and 35.00%in the control group,and the differences were statistically significant(P<0.05).Conclusion Combination of hepatectomy and fiber choledochoscopy shows remarkable effect in the treatment of hepatobiliary calculi,and it can effectively reduce the incidence of postoperative complications and residual stone rate and with high operation safety.It is worthy of clinical application.
