摘要
目的探讨耳穴贴压对胃癌根治术后胃肠功能及血清胃肠激素的调节作用。方法选择2014年1月～2017年7月在我院普外科拟行胃癌根治术患者70例,分为观察组和对照组各35例。两组患者均在全麻下行胃癌根治术,对照组术后给予围手术期常规治疗及干预,观察组在对照组基础上加用耳穴贴压治疗,持续1～2 min/次,按压5次/d,双耳交替进行。观察两组患者术后胃肠功能恢复时间,并比较术前与术后4 d血清胃动素及胃泌素水平的变化。结果观察组患者术后肠蠕动恢复时间、肛门排气和排便时间明显短于对照组(P<0.05);术后4 d,两组患者血清胃动素和胃泌素水平较术前均有不同程度上升(P<0.05或P<0.01),且治疗后观察组胃动素和胃泌素水平上升幅度高于对照组(P<0.05)。结论耳穴贴压用于胃癌根治术后的效果较肯定,可促进胃肠道蠕动,加快胃肠功能的恢复,其作用机制可能通过促进胃肠激素的分泌、提高血清胃动素和胃泌素水平、加快胃肠蠕动密切相关。
Objective To investigate the regulatory effect of auricular acupressure on the gastrointestinal function and serum gastrointestinal hormones after radical gastrectomy.Methods70patients undergoing radical gastrectomy from January2014to July2017in our Department of General Surgery were enrolled.The patients were divided into observation group and control group,with35cases in each group.Both groups underwent radical gastrectomy under general anesthesia.The control group was given conventional treatment and intervention during the perioperative period.The observation group was treated with auricular acupressure on the basis of the treatment of the control group for1-2min/time,with5times per day.Both ears were alternately pressed.The recovery time of gastrointestinal function after operation was observed in both groups,and the changes of serum motilin and gastrin before surgery and at4days after surgery were compared.Results The recovery time of the peristalsis,anal exhaust and defecation time were significantly shorter in the observation group than those in the control group after surgery(P<0.05).On the4th day after operation,the levels of motilin and gastrin in the observation group were increased to varying degrees compared with those before the operation(P<0.05or P<0.01).And the motilin and gastrin levels in the observation group after treatment were higher than those in the control group(P<0.05).Conclusion Auricular acupressure has positive effect in the treatment of gastric cancer after radicalresection.It can promote gastrointestinal motility and accelerate the recovery of gastrointestinal function.The mechanism of action may be to increase serum motilin and gastrin levels and to accelerate gastrointestinal motility by promoting the secretion of gastrointestinal hormones.
