Cost-Effectiveness Comparison of Different Dosage Regimens in the Treatment of Diabetic Peripheral Neuropathy

摘要目的比较不同药物治疗方案用于糖尿病周围神经病变(DPN)的成本-效果。方法将120例DPN患者按治疗方案的不同分为A组、B组和C组,各40例。A组患者给予硫辛酸,B组患者给予甲钴胺联合硫辛酸,C组患者给予依帕司他联合硫辛酸治疗,比较3组患者的临床疗效和不良反应发生情况,运用成本-效果分析法分析3组患者的成本-效果、增量成本-效果,并进行敏感度分析。结果 A组、B组、C组用药成本分别为1 717. 52元、2 135. 84元、1 973. 72元; A组总有效率为65. 00%,明显低于B组的87. 50%和C组的92. 50%(P <0. 05);成本-效果比A组为26. 42,B组为24. 41,C组为21. 34;敏感度分析支持成本-效果分析结果。3组不良反应发生率无明显差异(P> 0. 05)。从药物经济学角度分析,C组治疗方案较理想。结论硫辛酸联合依帕司他治疗DPN既经济又高效,值得临床医师首选。 Objective To compare the cost-effectiveness of 3 different dosage regimens in the treatment of diabetic peripheral neuropathy( DPN). Methods Totally 120 patients with DPN were randomly divided into group A,group B and group C according to different regimens. Group A was treated with lipoic acid,group B was treated with methycobal and lipoic acid,group C was treated with epalrestat combined with lipoic acid. The clinical efficacy and the adverse drug reactions were compared among the three groups. Cost-effectiveness analysis was used to analyze the cost-effectiveness ratio,the incremental cost-effectiveness ratio and the sensitivity analysis of the three groups. Results The cost of group A,group B and group C was 1 717. 52 Yuan,2 135. 84 Yuan and 1 973. 72 Yuan,respectively,and the total effective rate in group A was 65. 00%,which was significantly lower than 87. 50% in group B and 92. 50%in group C( P < 0. 05). Cost-effectiveness ratios were 26. 42,24. 41 and 21. 34 for group A,group B and group C,respectively. The results of cost-effectiveness analysis were supported by sensitivity analysis. There was no statistical significance in the incidence rate of adverse drug reaction among the three groups( P > 0. 05). In terms of pharmacoeconomics,the regimen of group C was the most cost-effective for the treatment of DPN. Conclusion Lipoic acid combined with epalrestat in the treatment of DPN is economical and efficient,which is worthy of the first choice of physicians.
