摘要
目的分析股骨头坏死患者血浆中硬化蛋白的含量变化,及其作为早期诊断标志物的可行性。方法纳入98名股骨头坏死患者(ARCO分期Ⅱ期10例、Ⅲ期38例、Ⅳ期50例)和96名健康成年人(对照组)。使用酶联免疫吸附法(ELISA)检测血浆硬化蛋白水平,并分析比较不同ARCO分期、病因、塌陷前后之间的硬化蛋白的水平差异。结果 (1)股骨头坏死患者的血浆硬化蛋白水平显着低于阴性对照组(P <0.001)。(2)激素性、酒精性、创伤性及特发性股骨头坏死病例之间无显著差异(P=0.359)。(3)不同ARCO分期的患者之间的硬化蛋白水平存在显著差异(P=0.039)。(4)ROC曲线分析提示硬化蛋白水平在诊断股骨头坏死塌陷时存在一定的敏感性和特异性(分别为59.09%,90.00%)。结论血浆硬化蛋白与股骨头坏死的发展过程关系密切,在股骨头坏死的早期诊断和病情监测中具有较高的参考意义,可为临床诊断、治疗方案治疗提供参考。
Objective To compare plasma sclerostin level in osteonecrosis of femoral head(ONFH)patients and in healthy controls and then to analyze the feasibility of sclerostin as a biomarker for early diagnosis of ONFH.Methods Ninety-eight ONFH patients including 10 ARCO stageⅡ,38 stageⅢand 50 stageⅣand 96 healthy controls were enrolled in the present study.Plasma sclerostin levels were assessed using an enzyme-linked immunosorbent assay development kit.The sclerostin levels were compared among different ARCO stages,etiology groups and collapse state.Results Plasma sclerostin levels were significantly lower in ONFH patients than those in healthy controls(P<0.001).In term of sclerostin level,there was no significant difference between steroid-induced ONFH cases and alcohol-abuse,trauma-induced and idiopathic ONFH cases(P=0.359).And significant differences were observed in sclerostin levels between ONFH patients with different ARCO stages(P=0.039).ROC curve analysis showed that sclerostin level had certain sensitivity and specificity in the diagnosis of collapse of ONFH(59.09%and 90.00%,respectively).Conclusion The plasma sclerostin closely relates to the develop-ment of ONFH and has a high reference value in the early diagnosis and monitoring of ONFH,which can provide reference for the clinical diagnosis and treatment.
