减数矫正开颌29例疗效观察

摘要 目的探讨拔牙矫正开颌后,患者垂直向间隙关闭情况以及患者颌骨面型、下颌平面的旋转变化情况,研究开颌改善的矫正机理。方法选取30例安氏一类开颌患者,均选择拔牙矫正,记录患者正常覆颌覆盖建立时间,每位患者分别拍摄治疗前后X线头颅侧位片,并进行头影测量,分析治疗前后SN-MP、FMA、PP-MP、OPMP、U1-SN、L1-MP、U6-PP、L6-MP以及AFH、ALFH等值的变化。结果 30例患者中29例治疗有效,建立了正常的覆颌覆盖,1例由于配合度不够,治疗失败。治疗后,U6-PP、L6-MP增大,FMA减小,改变有显著性差异,UI-OP,LI-MP略增大,SN-MP、PP-MP、OP-MP减小,AFH、ALFH增大,但无显著性差异。结论通过拔牙矫正开颌并不是通过前牙的伸长,而是依赖磨牙区颌骨高度的降低。 Objective To investigate the changes of vertical clearance and patient jaw type and mandibular plane rotation of open bite cases after extraction of teeth to correct maxillary opening,and to study the correction mechanism of maxillary opening improvement.Methods 30 patients were selected with class 1 open bite case,all of which choose correction of tooth extraction,lateral cephalometric films were taken before and after treatment.The figure of SN-MP,FMA,PP,MP,OP-MP,U1-SN,L1-MP,U6-PP,L6-MP and the change of AFH,ALFH are analyzed.Results 29 cases in 30 patients were effective,1 case was failed for lack of cooperation.After treatment,U6-PP,L6-MP increased,FMA decreased,and there were significant differences in the changes.UI-OP,LI-MP increase slightly,SN-MP,PP-MP,OP-MP decreased,AFH,ALFH increased,but there were no significant difference.Conclusion The correction of open bite is not by the front teeth tooth elongation,but depends on the reduction of maxillary height in the molar region.
