摘要
探讨和分析不同时期颅骨修补术对重型颅脑外伤去大骨瓣减压术患者的临床效果。选取我院2015年7月～2017年7月收治的重型颅脑外伤去大骨瓣减压术后患者98例,随机分为早期组52例和晚期组46例,早期组在去大骨瓣减压术后1个月～3个月行颅骨修补术,晚期组在术后6个月后行颅骨修补术,比较两组术后神经功能、脑组织血流变化和并发症发生情况。早期组患者Karnofsky (KPS)评分和功能独立性测量(FIM)评分明显优于对照组(P<0.05或P<0.01),患者患侧大脑中动脉(MCA)和颈内动脉(ICA)的血流速度明显高于晚期组(P<0.05),早期组颅骨修补术后颅内血肿、颅内感染、皮下积液和切口愈合障碍等并发症发生率明显低于晚期组(χ2=6.56,P<0.05)。重型颅脑外伤去大骨瓣减压术后早期完成颅骨修补术能明显改善神经功能和增加脑血液循环,减少并发症的发生,提高患者生命质量。
To investigate and analyze the clinical effects of cranioplasty in different periods on patients with severe craniocerebral trauma undergoing decompressive craniectomy 98 patients with severe craniocerebral trauma who were admitted to our hospital from July 2015 to July 2017,the patients were randomly divided into 52 cases in the early group and 46 cases in the late group.The early group underwent cranioplasty 1~3 months after the decompression of the large bone flap.Group of patients with early Karnofsky(KPS)score and functional independence measure(FIM)scores significantly better than that of control group(P<0.05 or P<0.01),patients with bilateral middle cerebral artery(MCA)and blood flow velocity of internal carotid artery(ICA)was obviously higher than that of the late group(P<0.05),the early group of skull repairing postoperative intracranial hematoma,intracranial infection,subcutaneous effusion and incision healing obstacles such as incidence of complications was lower than that in group late(χ^2=6.56,P<0.05).Early completion of craniocerebral repair after severe craniocerebral trauma decompression can significantly improve neurological function,increase cerebral blood circulation,reduce complications and improve the quality of life of patients.
