支气管肺泡灌洗术在肺曲霉菌病患者早期诊断中的应用 被引量:2

摘要 目的探究支气管肺泡灌洗术(BAL)在肺曲霉菌病患者早期诊断中的应用。方法选取2014年6月~2018年6月本院收治肺部感染患者中的60例,对所有患者进行BALF收集送检GM实验,并根据结果分为病例组、对照组。统计分析各组GM试验的结果。结果 GM1亚组、GM2亚组检测GM值上存在有显著差异,GM1亚组GM值相对较高(P <0.05)。且BAL诊断的敏感度、特异度、阳性预测值等均相对较高。BALF-GM1曲线的下面积相比于BALF-GM2显著更高。结论在支气管肺泡灌洗术中,BALF收集操作顺序对于BALF-GM试验的I值存在一定的影响,同时送检第1管的肺泡灌洗液BALF-GM试验对于肺曲霉病的诊断上具较高诊断价值。 Objective To investigate the application of bronchoalveolar lavage(BAL)in the early diagnosis of patients with pulmonary aspergillosis.Methods Sixty patients with pulmonary infection admitted to our hospital from June 2014 to June 2018 were randomLy selected.All patients were subjected to BALF collection and GM test,and were divided into case group and control group according to the results.The results of each group of GM trials were statistically analyzed.Results There were significant differences in the GM values between the GM1 subgroup and the GM2 subgroup.GM values in the GM1 subgroup were relatively high(P<0.05).The sensitivity,specificity,and positive predictive value of BAL diagnosis were relatively high.The area under the BALF-GM1 curve was significantly higher than that of BALF-GM2.Conclusion In the bronchoalveolar lavage,the BALF collection sequence has a certain effect on the I value of the BALF-GM test,and the BALF-GM test for the first tube of the alveolar lavage fluid has a higher diagnostic value for pulmonary aspergillosis.
