摘要
目的分析住院慢性阻塞性肺疾病合并肺动脉高压患者证型分布及主要检验指标情况。方法采用回顾性研究方法,通过查阅2015年1月至2015年12月期间中国中医科学院西苑医院肺病科住院的197例慢性阻塞性肺疾病合并肺动脉高压患者病历获得资料,记录患者主要临床症状、中医证型及主要检验指标等进行分析。结果主要症状为喘息憋气;主要证型为痰热蕴肺证,肺功能FEV1平均值为(55.7±10.7)%;动脉血氧分压平均值为(72.1±16.3)cmH2O;白细胞计数平均值为(8.0±3.3)×109/L;中性粒细胞百分比平均值为(71.2±12.9)%;C反应蛋白平均值为(45.4±84.6)mg/L;BNP平均值为(942.2±3 030.7)pg/mL;D-二聚体平均值为(1.3±4.7)mg/L;纤维蛋白原浓度平均值为(3.7±1.2)g/L;肺动脉收缩压平均值为(44.5±9.6)mmHg。结论本病主要症状是喘息憋气,主要证型为痰热蕴肺证,"毒-瘀"贯穿于本病的发展,治疗本病时要重视解毒(炎症)祛瘀(治本)的治疗原则,重视解毒祛瘀中药的抗炎作用。
Objective To analyze investigate the distribution of the syndrome type and the main index of the patients with chronic obstructive pulmonary disease(COPD)combined with pulmonary hypertension(PH).Methods Use retrospective studyone hundred and ninety-seven,197 cases of COPD combined with PH patient records from January 2015 to December 2015 by referring to in the Department of respiration of Xiyuan Hospital of CACMS were enrolled in this study.The clinical symptoms,TCM syndrome,and main inspection indexes were collected and analyzed.Results The main symptoms of COPD combined with PH were cough and expectoration,andthe main syndrome type was phlegm heat accumulation of lung syndrome.,The average FEV1 pulmonary function value was(55.7±10.7)%,with PaO2 average of(72.1±16.3)cmH2O.Blood cell count average value was(8.0±3.3)×10^9/L,neutrophil percentage of average value was(71.2±12.9)%,with C reactive protein with an average of(45.4±84.6)mg/L.The average value of BNP was(942.2±3 030.7)pg/mL,two D-dimer average value was(1.3±4.7)mg/L,with the concentration of fibrinogen with an average of(3.7±1.2)g/L,and the pulmonary artery systolic pressure with an average of(44.5±9.6)mmHg.Conclusion The main symptoms of COPD combined with PH were cough and expectoration,and the main syndrome type was phlegm heat accumulation of lung syndrome.“poison-blood stasis”runed through the development of this disease,so the principle of treating this disease should be detoxification(based on the inflammation)and remove blood stasis(based on the basic cause of this disease),paying attention to the anti-inflammatory effect of detoxification and removing stasis of the traditional chinese medicine.
