摘要
目的观察腹腔镜联合胆道镜下钬激光碎石术治疗胆管结石的临床效果。方法 140例胆管结石患者,采取随机抽样法分为观察组与对照组,各70例。对照组患者采取传统开腹联合胆道镜常规取石术治疗,观察组患者采取腹腔镜联合胆道镜下钬激光碎石术治疗,对比两组患者的取石彻底率、手术时间、住院时间、再次手术率及并发症发生情况。结果观察组患者取石彻底率高于对照组,再次手术率低于对照组,手术时间短于对照组,差异均有统计学意义(P<0.05)。观察组患者并发症发生率为2.86%,明显低于对照组的17.14%,住院时间为(8.84±1.58)d,明显短于对照组的(13.89±1.36)d,差异具有统计学意义(P<0.05)。结论胆管结石患者实施腹腔镜联合胆道镜下钬激光碎石术具有更好的取石效果,结石清除效果明显,再次手术率低,且能够降低术后并发症风险,缩短住院时间,值得临床应用。
Objective To observe the clinical effect of laparoscopy combined with holmium laser lithotripsy under choledochoscope for the treatment of calculus of bile duct.Methods A total of 140 patients with calculus of bile duct were divided by random sampling method into observation group and control group,with 70 cases in each group.The control group received traditional laparotomy combined with choledochoscopy,and the observation group received laparoscopy combined with holmium laser lithotripsy under choledochoscopy.Comparison were made on stone removal rate,operation time,hospitalization time,reoperation rate and occurrence of complications between the two groups.Results The observation group had higher stone removal rate than the control group,lower reoperation rate and shorter operation time than the control group.Their difference was statistically significant(P<0.05).The observation group had obviously lower incidence of complications as 2.86%than 17.14%in the control group,and obviously shorter hospitalization time as(8.84±1.58)d than(13.89±1.36)d in the control group.Their difference was statistically significant(P<0.05).Conclusion Implementation of laparoscopy combined with holmium laser lithotripsy under choledochoscope shows better stone removal effect for patients with calculus of bile duct with obvious stone removal effect and low reoperation rate,and it can reduce the risk of postoperative complications and shorten hospitalization time.It is worthy of clinical application.
