摘要
目的探讨B细胞非霍奇金淋巴瘤(B-cell non-Hodgkin lymphoma,B-cNHL)患者应用化疗药物联合或不联合利妥昔单抗(rituximab,RTX)治疗后发生间质性肺炎(interstitial pneumonia,IP)的相关因素。方法分析92例Bc NHL患者中可能影响IP发生的各种临床因素,以明确引起IP发生的高危因素。结果 11例B-cNHL患者发生了IP,总发生率为11. 96%;单用化疗药物组共41例,其中1例发生IP,发生率为2. 44%; RTX联合化疗药物组共51例,其中10例发生IP,发生率为19. 61%。分析发现只有化疗联合RTX这一治疗方案是影响IP发生的独立危险因素(P <0. 05,OR=9. 613)。生存分析进一步比较化疗联合RTX组和单用化疗药物组IP的发病率,发现前组IP的发生率高于后组,具有统计学意义(P=0. 011)。结论化疗药物联合RTX方案是B-cNHL治疗相关IP的独立危险因素,临床医生在应用该方案过程中应提高警惕。
Objective To explore the related factors of interstitial pneumonia(IP)in B cell non-hodgkin's lymphoma(B-cNHL)patients who were treaded by chemotherapeutic drugs with rituximab(RTX).Methods The various clinical factors that may affect the occurrence of IP in 92 patients with B-cNHL were analyzed to determine the risk factors for the occurrence of IP.Results The incidence of IP in 11 B-cNHL cases was 11.96%.41 cases were treated with chemotherapy alone,and 1 case had IP,the incidence was 2.44%.51 cases were treated by RTX with chemotherapy drugs,and 10 cases had IP,the incidence rate was 19.61%.It was found that chemotherapy combined with RTX was the independent risk factor for the occurrence of IP(P<0.05;OR=9.613).Finally,survival analysis further compared the incidence of IP in the chemotherapeutic drugs with RTX group and chemotherapeutic drugs group,and the results found the incidence of IP of the former was markedly higher than the latter(P=0.011).Conclusion The combination of chemotherapeutic drugs and RTX is independent risk factor for the treatment of IP.Doctors should be careful during treatment.
