摘要
目的观察依帕司他、甲钴胺联合胰岛素泵治疗糖尿病周围神经病变(Diabetic perineuropathy,DPN)的临床疗效。方法本研究选取2017年以后在我科住院治疗的糖尿病周围神经病变患者102例;根据随机数表法分为观察组及对照组各51例;观察组选用依帕司他、甲钴胺联合胰岛素泵进行治疗,对照组选用甲钴胺联合胰岛素泵进行治疗;治疗周期为一个月;观察两组患者治疗前后的运动神经传导速度(Motor nerve conduction velocity,MNCV)、感觉神经传导速度(Sensory nerve conduction velocity,SNCV)、多伦多临床评分系统(Toronto clinical scoring system,TCSS)的评分情况及临床疗效。结果观察组与对照组治疗前后正中神经及腓总神经MNCV、SNCV均较治疗前明显改善(P<0.05),治疗后观察组正中神经和腓总神经的MNCV、SNCV较对照组比明显改善(P<0.05);观察组与对照组治疗前后TCSS评分均明显降低(P<0.05),治疗后观察组TCSS评分较对照组明显降低(P<0.05);治疗后两组患者临床疗效比较差异显著(P<0.05)。结论依帕司他、甲钴胺联合胰岛素泵治疗糖尿病周围神经病变疗效确切,联合用药较单药治疗可明显提高临床治疗效果。
Objective To observe the clinical effect of epalrestat,mecobalamin combined with insulin pump in the treatment of diabetic peripheral neuropathy(Diabetic perineuropathy,DPN).Methods 102 patients with DPN hospitalized in our department after 2017 were selected in this study;According to the random number table,they were divided into the observation group and the control group,51 cases in each group;The observation group was treated with epalcitazone and megalobalamin combined with insulin pump,while the control group was given mecobalamin combined with insulin pump;The treatment cycle was one month;The motor nerve conduction velocity(MNCV),the sensory nerve conduction velocity(SNCV),scores of Toronto clinical scoring system(TCSS)and the clinical efficacy of the two groups before and after treatment were observed.Results Compared with before treatment,the MNCV and SNCV of median nerve and common peroneal nerve in the observation group and the control group were significantly improved before and after treatment(P<0.05),and the MNCV and SNCV of median nerve and common peroneal nerve in the observation group was significantly improved than that in the control group after treatment(P<0.05);The TCSS scores of the observation group and the control group were significantly decreased before and after treatment(P<0.05),and the TCSS score in the observation group was significantly lower than that in the control group after treatment(P<0.05);There was a significant difference in clinical efficacy between the two groups after treatment(P<0.05).Conclusion The curative effect of epalrestat,mecobalamin combined with insulin pump in the treatment of DPN was definite.And the combination of drugs can significantly improve the clinical effect than a single drug treatment.
