血小板反应蛋白1(THBS-1)与急性缺血性脑卒中诊断和预后的相关性研究 被引量:14

摘要 目的应用蛋白质组学技术检测和鉴定对急性缺血性脑卒中(acute ischemic stroke,AIS)患者有诊断和预后评估作用的潜在生物标志物,为缺血性脑卒中的临床管理提供新的思路和方法。方法收集AIS患者40例(脑卒中1组)及同期健康志愿者20例(对照1组)的血浆样本,应用蛋白组学技术鉴定脑卒中1组和对照1组的差异表达蛋白;前瞻性纳入AIS患者33例(脑卒中2组),同期健康志愿者33例(对照2组),利用Western blot方法检测差异表达蛋白血小板反应蛋白1(thrombospondin-1,THBS-1)在脑卒中2组和对照2组血浆中的表达水平。根据THBS-1诊断急性缺血性脑卒中的能力,绘制ROC曲线,计算曲线下面积。用Logistic回归分析探讨THBS-1表达水平与患者预后的相关性。结果脑卒中1组和对照1组的血清蛋白质组学分析共得出7种差异表达蛋白,其中THBS-1在脑卒中1组的表达量比对照1组升高1.46倍(P<0.05),为上调蛋白。应用Western blot验证发现THBS-1在脑卒中2组的表达量比对照2组高(P<0.01),这一结果与蛋白质组学分析一致。根据THBS-1诊断AIS的能力绘制ROC曲线,计算曲线下面积为0.7686,95%CI为0.6521~0.8851,P=0.0001,提示THBS-1可能是AIS患者早期诊断的可靠生物标志物。以Logistic回归分析对THBS-1表达水平和患者预后进行相关性探讨,OR=3.294,95%CI为1.158~9.372,P<0.05,提示血浆THBS-1表达水平和急性缺血性脑卒中的预后之间存在相关性,且THBS-1表达量越高,患者预后越差。结论急性缺血性脑卒中患者血浆中高表达THBS-1,且THBS-1的表达水平越高,患者预后越差,提示检测THBS-1可有助于急性缺血性脑卒中的早期诊断和预后评估。 Objective To examine the value of thrombospondin 1(TH BS-1.)for the diagnosis and prognosis of acute ische mic strokcC AIS.)by using the protcomic technique in order to provide a new idea and method for the clinical management of AIS.Methods Forty AS paticnts(stroke group 1.)and 20 healthy voluntccrs(control group 1.)were collected to identify the dif-fcrcntially expressed proteins by the protcomic tcchniqu..In addition,3 AIS patientsCstroke group 2.)and 33 healthy volunteers(control group 2.)were prospectively enrolled.Western blotting was used to detect the expression of THBS-1 in stroke group 2 and control group 2.According to the ability of THBS-1 to diagnose AIS,the ROCcurve was drawnand the arcaundcr the curve calculated.The correlation between TH BS-1 expression and prognosis was analyzed by Logistic regression analysis.Results A total of 7 differentially expressed proteins were identified in stroke group 1 and control group 1 by the scrum protcomic analysis.The expression of TH BS-1 in stroke group 1was.4 6 times higher than that in control group 1(P〈0.05.).Western blotting showed that the expression of THBS-1 in stroke group 2 was higher than that in control group 2(P〈0.01),which was consistent with the protcomic resut.The ROC curve showed that the area under the curve was 0.7686,and the 95%Cl was 0.6521-0.8851(P=0.0001),which suggested that THBS-1 was a reliable biomarkcr for early diagnosis of AIS.Logistic regression analysis showed that OR was 3.294 and the 95%CI was.1 58-9.372(P〈0.05),indicating that there was a correlation between the expression of THBS-1 and the prognosis of AIS patients.The higher the expression of TH-BS-1,theworse the prognosis ofAIS patients Conclusion THBS-1 was found to be highly expressed in the plasma of AS patients,and the higher the expression level ofTH BS-1,the worse the prognosis t is suggested thatTH BS-1 may assist in the early diagnosis and prognosis evaluation of AIS.
