摘要 目的:探讨不同程度脑白质疏松患者的步态和平衡障碍的特点。方法:收治脑白质疏松(LA)患者172例,根据Fazekas量表,分1~3级。收集临床资料,评价共济运动及步态和平衡功能。结果:Fazekas量表的分级越高,步态和平衡功能损害越严重,1级组只有少数患者出现手部的动作缓慢,没有步态和躯体的障碍,在2~3级组除了手部的精细动作缓慢,还出现了躯体少动、速减慢、步基增宽、步长不对称、360°转身、瞬间的站立不稳定等运动障碍。结论:LA分级越高,步态和平衡功能损害越严重。步态障碍主要表现为步速减慢、拖地、步基宽和步长不对称。步态和平衡功能障碍的早期评价对于预防跌倒有重要意义。 Objective:To explore the characteristics of gait and balance disorders in patients with different degrees of leukoaraiosis.Methods:172 cases of LA patients were selected,according to the Fazekas scale,they were divided into 1~3 levels,we collected clinical data to assess mutual movement and gait and balance.Results:As the grading of Fazekas scale increased,the gait and balance function impairment was more serious.In the 1 grade group,only a few patients had slow movements of hand,no gait and somatic disorder.In the 2nd to 3rd grades,in addition to the slow movement of the hand,there were also movement disorders such as less movement of the body,slower speed,wider step base,asymmetrical step length,360°turn,and unstable standing instability.Conclusion:The higher the LA classification,the more serious the gait and balance function impairment.Gait disorders are mainly slowed down by pace,mop the floor,step width and step asymmetry.Early assessment of gait and balance dysfunction is important for preventing falls.
