摘要
目的探讨快速康复外科(FTS)在三甲医院腹部外科中的应用现状和实施障碍。方法选取2 300名来自全国31所三甲医院腹部外科的医护人员为研究对象,利用自制问卷对FTS的应用现状、实施障碍进行调查,并分析影响应用现状和实施障碍的主要因素。结果单因素分析结果显示:不同科室、是否参加过FTS培训与FTS的开展应用情况有关,差异有统计学意义(P<0.05)。实施FTS面临的主要障碍因素为:患者病情不同,不能千篇一律地应用(84.30%);推广和普及程度不够(76.22%);缺乏多学科合作团队(73.54%);未形成具体疾病的临床指南(72.49%);医护人员缺乏深入学习和了解(71.76%)。结论倡导的优化措施部分应用于国内三甲医院的腹部外科,全面推广应用仍面临诸多障碍。卫生主管部门可通过制定具体疾病的FTS临床指南、组建多学科合作团队、加强医护人员培训,开展前瞻性研究全面验证其安全性等策略,以促进FTS在国内的普及应用。
To study the current status and barriers of implementing fast-track surgery in grade-III class-A hospitals,and to provide scientific evidences for developing policies for health authorities.Methods Multi-stage stratified sampling method was performed and 2300 medical staff from 31 grade-III class-A hospitals was involved in this survey.A self-designed questionnaire was used for the survey.Results Barriers of implementing fast-track surgery mainly including:each patient's condition is specific,fast-track programs can not be stereotyped implementation(84.30%);the popularization degree of fast-track surgery is not enough(76.22%);there is lack of multimodality therapy team(73.54%);there has not formed the specific disease clinical guidelines(72.49%);medical staff do not have in-depth study and understanding of fast-track surgery(71.76%).Conclusions Fast-track surgery has just been partially adapted to daily routine in abdominal surgery of grade-III class-A hospitals.Some elements needed to be improved.In order to promote the application of fast-track surgery in China,the health authorities can formulate clinical guideline of fast-track surgery,establish a multidisciplinary team,strengthen the training of medical staff,carry out prospective study to verify the safety.
