摘要
目的探讨促黑激素皮质素受体4(MC4R)-rs6567160位点基因多态性在代谢综合征(MS)发生、发展中的作用。方法选取MS患者520例(MS组)、体检健康者704例(对照组),检测两组MC4R-rs6567160位点TT、CC、TC三种基因型分布频率,分析TT、CC、TC三种基因型患者腰围、BMI及血糖、糖化血红蛋白(Hb A1c)水平及MC4R-rs6567160位点基因型与MS的关系。结果 MS组MC4R-rs6567160位点TT基因型分布频率低于对照组,CC基因型分布频率高于对照组,组间比较P均<0. 01。与TT基因型及TC基因型者比较,CC基因型者腰围和BMI明显升高(P均<0. 01);与TT基因型及CC基因型者比较,TC基因型者FPG及Hb A1c明显升高(P均<0. 05)。MC4R-rs6567160位点基因型是MS发病的危险因素(OR=1. 385,95%CI:1. 016~1. 889,P <0. 05),其中TC基因型发生MS的概率是TT基因型的1. 333倍(OR=1. 333,95%CI:1. 037~1. 713,P <0. 05),是CC基因型的2. 661倍(OR=2. 661,95%CI:1. 549~4. 571,P <0. 01)。结论 MC4R-rs6567160位点基因多态性是MS发生的独立危险因素,携带MC4R-rs6567160位点CC基因型的人群更易发生MS。
Objective To explore the role of single nucleotide polymorphisms(SNP)of melanocortin-4 receptor(MC4R)-rs6567160 in the occurrence and development of metabolic syndrome(MS).Methods Totally 520 patients with MS(MS group),and 704 healthy subjects in the same period(control group)were selected.The frequencies of TT,CC and TC of of MC4R-rs6567160 in the two groups were detected.The levels of waist circumference,body mass index(BMI),blood glucose,and glycosylated hemoglobin A1c(HbA1c),and MC4R-rs6567160 genotype were analyzed in pa-tients with TT,CC,or TC genotypes,as well as the relationship with MS.Results The frequency of TT genotype of MC4R-rs6567160 in the MS group was lower than that of the control group,and the frequency of CC genotype was higher than that of the control group.Compared with TT type and TC type,the waist circumference and BMI of patients with CC type increased(P<0.01).Compared with TT type and CC type,the fasting blood-glucose(FPG)and HbA1c of patients with TC type increased(both P<0.05).The MC4R-rs6567160 allele was a risk factor for MS(OR=1.385,95%CI:1.016-1.889,P=0.039),and the probability of MS occurrence in patients with TC was 1.333 times higher that of pa-tients with TT type(OR=1.333,95%CI:1.037-1.713,P<0.05),was 2.661 times higher than that of patients with CC type(OR=2.661,95%CI:1.549-4.571,P<0.01).Conclusion The MC4R-rs6567160 gene polymorphism is an in-dependent risk factor for MS,and the population carrying the MC4R-rs6567160 CC genotype is more prone to MS.
