摘要
目的:建立适用于优秀运动员的常用血液生化指标的参考范围。方法:国家队运动员共1 632名,清晨抽取静脉血5 ml,对红细胞(RBC)、白细胞(WBC)、血红蛋白(Hb)、红细胞压积(Hct)、血小板(Plt)、血尿素氮(BUN)、肌酸激酶(CK)、皮质醇(Cort)和睾酮(Testo)9项指标进行检测,用SPSS 18.0对数据进行统计分析,用参数法或非参数法求得其95%参考范围,得出优秀运动员以上指标的实验室参考范围,并将其与临床参考值进行比较;选取n≥40的项目进行分项目统计,用参数法或非参数法求得其95%参考范围。结果:1)本研究所有指标均具有性别差异;2)成功建立了中国优秀运动员部分血液生化数据的实验室参考范围,与临床参考值相比,血细胞分类的5项指标与临床范围并无明显差异,但BUN、CK、Cort和Testo明显高于临床参考范围;3)建立了男运动员举重,拳击、体操等5个项目及女运动员花样游泳、摔跤、体操等5个项目的实验室参考范围。结论:通过大样本量的统计分析,建立了中国优秀运动员的WBC、RBC、Hb、Hct、Plt、BUN、CK、Cort和Testo9项生化指标的实验室参考范围,以及男、女运动员各5个单项的实验室参考范围,为实验室检测工作中异常值的判定提供了必要的参考依据。
Objective:To establish the reference range for hematological indicators in elite athletes of China.Method:Hematological indicators,including white blood cells(WBC),red blood cells(RBC),hemoglobin(Hb),hematocrit(Hct),platelet(Plt),blood urea nitrogen(BUN),creatine kinase(CK),cortisol(Cort),testosterone(Testo)were assayed in whole blood and serum samples from 1632 athletes(884 male athletes and 748 female athletes)who visited the laboratory for biochemical monitoring during 2009-2017,and reference range were calculated by the parametric method(M±1.96s)or the non-parametric method(2.5th to 97.5th percentile).On the other hand,specific reference range of some sports which had more than 40 subjects was calculated by the above method.Results:1)There are gender differences of all hematological indicators;2)we succeed in establishing the reference range for hematological indicators in excellent athletes,some indicators such as BUN,CK,Cort,Testo are higher than clinical reference value;3)we established specific reference range of five men’s sports including weightlifting,boxing,gymnastics and so on,and five women’s sports of wrestling,synchronized swimming and gymnastics.Conclusion:Sport training and competition have profound effects on the reference range for hematological indices.Introducing sport-specific reference intervals may help to avoid misjudgment of abnormal values and make the test results more accurate.
