摘要
目的:探讨肺炎球菌DNA负荷(Lyt A基因)、降钙素原(PCT)和可溶性尿激酶型纤溶酶原激活物受体(su PAR)水平与肺炎链球菌感染患者病情严重程度的相关性。方法:选择184例肺炎链球菌感染患者作为研究对象,按照病情严重程度将患者分为轻度组(71例)、中度组(68例)和重度组(45例)。采用定量荧光PCR反应测定血液Lyt A基因,采用酶联免疫吸附法(ELISA法)检测血浆su PAR水平,采用量免疫色谱法检测PCT水平,根据肺炎严重指数评分对患者病情进行分度。观察Lyt A、PCT及su PAR水平与疾病严重程度的相关性。结果:3组患者Lyt A阳性率相比较差异无统计学意义(F=1.900,P>0.05);Lyt A、PCT及su PAR水平随着病情分度增加逐渐升高,3组比较差异均有统计学意义(F=62.057,F=73.892,F=48.509;P<0.05)。相关性分析显示,Lyt A、PCT及su PAR水平与病情严重程度均呈显著正相关(r=0.472,r=0.506,r=0.438;P<0.05);Lyt A与PCT、su PAR水平呈显著正相关(r=0.603,r=0.682;P<0.05)。结论:肺炎链球菌肺炎患者血液中lyt A、PCT及su PAR水平与病情严重程度均呈显著正相关;Lyt A基因量与PCT、su PAR水平呈显著正相关。
Objective:To study the correlation between DNA load,procalcitonin and soluble urokinase plasminogen activator receptor(suPAR)levels of pneumococcus and the infection degree of streptococcus pneumoniae.Methods:184 patients with streptococcus pneumoniae infection were divided into mild group(71 cases),middle group(68 cases)and severe group(45 cases)according to the severe degree of illness.The fluorescence quantitative PCR was adopted to detect blood LytA gene,and ELISA was adopted to detect plasma suPAR level,and immunochromatography was adopted to detect PCT level,and the illness state of patients were graded as severe index of pneumonia.And then,the correlation of LytA,PCT and suPAR with severe degree of illness were observed.Results:There was no significant difference in the positive rate of LytA among the three groups(F=1.900,P>0.05).The level of LytA,PCT and suPAR enhanced gradually with the increase of severe degree of disease,and the differences among groups were statistically significant(F=62.057,F=73.892,F=48.509,P<0.05).The correlation analysis showed that the levels of LytA,PCT and suPAR were significantly positive correlation with the severe degree of disease(r=0.472,r=0.506,r=0.438,P<0.05).And the LytA was significantly positive correlation with the levels of PCT and suPAR(r=0.603,r=0.682,P<0.05),respectively.Conclusion:The blood levels of LytA,PCT and suPAR of patients with pneumococcal pneumonia were significantly positive correlation with the severe degree of the disease.And the gene amount of LytA was significantly positive correlation with the levels of PCT and suPAR.
