摘要
目的评价多发性骨髓瘤(MM)^(18) F-FDG PET/CT的影像表现与临床预后参数的关系。方法回顾性分析在该院行^(18)F-FDG PET/CT检查的33例MM初诊患者。收集影像参数包括PET和CT所检测到的病灶总数、有活性的病灶数目、病灶的代谢参数最大标准摄取值(SUVmax),以及临床预后参数包括血清血钙、血红蛋白、肌酐、乳酸脱氢酶、β2微球蛋白、清蛋白、骨髓浆细胞比例、D-S分期、ISS分期。结果 PET/CT表现较差即活性病灶数较多的患者发生高钙血症、血清β2微球蛋白升高、D-S分期为Ⅲ期患者所占比例均高于活性病灶数较少的患者(P<0.05)。而两组患者年龄、性别,以及发生低蛋白血症、贫血、肾功能受损、血LDH增高、骨髓浆细胞大于10%、ISS分期为Ⅲ期所占比例差异均无统计学意义(P>0.05)。不计病灶代谢活性,以单纯病灶数和SUVmax分别进行分组,上述各项临床参数差异均无统计学意义(P>0.05)。结论 ^(18) F-FDG PET/CT所提供的活性病灶数较单纯的SUVmax和单纯的病灶数能更准确反映MM的生物学特点。
Objective To evaluate the relationship between 18 F-FDG PET/CT image parameters and clinical prognostic parameters in the patients with multiple myeloma(MM).Methods A total of 33 cases of newly diagnosed MM undergoing 18 F-FDG PET/CT examination in the hospital were retrospectively analyzed.The collected image parameters included the total number of lesions,active lesions and lesion metabolic parameter maximum of standard uptake value(SUVmax).The clinical prognostic parameters included serum calcium,hemoglobin,creatinine,lactic dehydrogenase(LDH),β2 microglobulin,albumin,plasma cell percentage in bone marrow,Durie-Salmon(D-S)stage and International Staging System(ISS)stage.The Fisher′s exact test was used to compare the difference of clinical parameters among different PET/CT parameters.Results The proportions of the patients with poor PET/CT manifestation,i.e.,more active lesions,developing hypercalcemia,β2-microglobulin increase and D-S stageⅢwere higher than those in the patients with less active lesions(all P<0.05).But the age,sex,and proportions of hypoproteinemia,anemia,azotemia,hyperlactate dehydrogenase,bone marrow plasmocytes>10%and ISS stageⅢhad no statistical differences between the two groups(P>0.05).Disregarding the lesion metabolic activity,the grouping was performed according to simple lesion number and SUVmax,the differences of above clinical parameters had no statistical significance(P>0.05).Conclusion The active lesions number provided by 18 F-FDG PET/CT can more accurately reflect the MM biological characteristics compared with single SUVmax and single lesion number.
