摘要
目的了解上海市2013—2015年所有参与社区卫生服务中心家庭医生制度建设工作的机构进展情况,找到问题并提出政策建议。方法对2013—2015年上海家庭医生制度的建立和发展趋势进行描述性分析。结果上海市家庭医生制度(1.0版)的构建始于2010年底,至2015年底在全市范围基本全覆盖。在此期间,家庭医生及其团队数量和质量缓慢提升,常住居民签约率稳步上升,初级卫生服务逐步推进,预约和双向转诊的数量逐渐增加,健康管理服务方面成绩显著。但仍存在团队人员配备不合理、远郊居民签约率下降、城区家庭医生签约饱和率低、健康档案动态更新率低等问题。结论上海市家庭医生制度(2.0版)需深入分析城郊差异,提升健康管理服务内涵,加大对家庭医生岗位的政策倾斜力度,并积极落实签约工作,以促进有序诊疗。
Objective To understand status and development of the Shanghai family doctor system from 2013 to 2015,find problems and put forward policy suggestions.Methods Descriptive analysis on establishment and developing trend of the family doctor system in Shanghai from 2013 to 2015 were made.Results The Shanghai family doctor system(version 1.0)began at the end of 2010 and was basically covered all over the city by the end of 2015.During this period,the quality of family doctors and the team slowly improved and its quantity also increased slowly,permanent residents signing rate rose steadily,and the primary health service was carried out smoothly,meanwhile the quantity of appointment and two-way referral gradually increased,health management service have had remarkable achievements.However,there were still some problems such as unreasonable team staffing,declining sign-up rate of distant suburbs and low dynamic updating of health records.Conclusion It is suggested to improve the content of health management services deeply,provide more preferential policy to family doctors,and actively implement contract work in the Shanghai family doctor system(version 2.0)to promote orderly diagnosis and treatment.
