摘要
目的:分析肺癌临床诊断中应用PET/CT检查的价值与结果。方法:回顾性分析2016年6月-2017年12月本院收治的可疑肺部恶性病变患者88例的临床资料。患者均实施组织病理学检查和PET/CT检查,以病理学检查为金标准,比较肺部良性病变与肺癌的^(18)F-FDG摄取差异,并总结分析PET/CT检查误诊原因。结果:经组织病理学检查,本组88例患者中确诊为肺部良性病变者20例(22.73%)、肺癌者68例(77.27%),经PET/CT检查确诊为肺部良性病变者13例(14.77%)、肺癌者67例(76.14%),其中1例阳性假瘤(炎性假瘤),3例结核、3例机化性肺炎被误诊为肺癌,1例中央型肺癌误诊为肺炎肺不张。PET/CT检查肺癌的特异度为65.0%(13/20)、敏感度为98.5%(67/68)。经PET/CT检查,肺良性病变组织SUV_(max)(7.25±3.03)低于肺癌组织的(10.68±5.47),比较差异有统计学意义(P<0.05)。结论:在临床诊断肺癌的过程中,采用PET/CT检查是一种综合诊断方式,且与肺良性病变组织相比,肺癌组织经PET/CT检查的^(18)F-FDG摄取值较高,而造成诊断结果假阳性较高的原因主要为少数机化性肺炎和结核的PET/CT表现与肺癌类似。
To analyze the value and results of the application of PET/CT in the clinical diagnosis of lung cancer.Method:The clinical data of88patients with suspicious pulmonary malignancy who admitted to our hospital from June2016to December2017were retrospectively analyzed.All patients underwent histopathological examination and PET/CT.The pathological examination was the gold standard,the difference of18F-FDG uptake between lung benign lesions and lung cancer was compared,and the misdiagnosis causes of PET/CT examinations were analyzed.Result:According to histopathological examination,there were20cases(22.73%)diagnosed as benign lung lesions and68cases(77.27%)diagnosed with lung cancer in88cases.According to PET/CT examination,13cases(14.77%)diagnosed as benign lung lesions and67cases(76.14%)diagnosed with lung cancer,among them,1case of positive pseudotumor,3cases of tuberculosis and3cases of systemic pneumonia were misdiagnosed as lung cancer,and1case of central lung cancer was misdiagnosed as pneumoconiosis.The specificity of PET/CT for lung cancer was65.0%(13/20)and sensitivity was98.5%(67/68).After PET/CT examination,the SUVmax in lung benign lesions was(7.25±3.03),which was significantly lower than(10.68±5.47)in lung cancer tissues,the difference was statistically significant(P<0.05).Conclusion:PET/CT is a comprehensive diagnostic method in the clinical diagnosis of lung cancer,compared with benign lung tissue,lung cancer tissue underwent PET/CT examination with significantly higher18F-FDG uptake,and the reason for the higher false positives was mainly due to the similar PET/CT performance of a few institutionalized pneumonias and tuberculosis.
