摘要
目的研究实时超声弹性成像及灰阶检查超声在甲状腺占位性病变诊断的对比效果。方法收集本院收治的86例甲状腺占位性病变患者,收集时间是自2015年11月~2017年11月,分试验组和对照组,每组43例。对照组采纳灰阶超声检查,试验组采纳实时超声弹性成像检查。比较检查结果,并与病理结果作对比。结果 86例患者经病理检查示:共发现病灶161个,其中52个恶性病灶,占32.3%;109个良性病灶,占67.7%。试验组对于良性病灶和恶性病灶的诊断准确率显著比对照组的高,差异有统计学意义(P<0.05);准确度、特异度和敏感度:试验组的分别是98.1%、100.0%、94.2%,对照组的分别是77.6%、78.9%、75.0%,差异有统计学意义(P<0.05);准确度、特异度和敏感度试验组的显著较高,差异有统计学意义(P<0.05)。实时超声弹性成像诊断中,良性病灶109例,其中12例是0级、21例是Ⅰ级、53例是Ⅱ级、17例是Ⅲ级、6例是Ⅳ级。恶性病灶49例,7例是Ⅱ级、19例是Ⅲ级、23例是Ⅳ级。结论甲状腺占位性病变患者采纳实时超声弹性成像检查,诊断准确率较高,对患者的占位性病变部位做出进一步明确和诊断,值得借鉴。
Objective To study the contrast effect of real-time ultrasound elastography and gray-scale examination of ultrasound in the diagnosis of thyroid occupying lesions.Methods Eighty-six patients with thyroid space occupying lesions admitted to our hospital were collected from November 2015 to November 2017.They were divided into experimental group and control group,43 cases in each group.The control group adopted gray-scale ultrasonography,and the experimental group adopted real-time ultrasound elastography.Compare test results and compare with pathology results.Results The pathological examination of 86 patients showed that there were 161 lesions,including 52 malignant lesions,accounting for 32.3%;109 benign lesions,accounting for 67.7%.The diagnostic accuracy of benign lesions and malignant lesions in the experimental group was significantly higher than that in the control group(P<0.05).Accuracy,specificity,and sensitivity:98.1%,100.0%,and 94.2%in the test group,and 77.6%,78.9%,and 75.0%in the control group,respectively.The difference was statistically significant(P<0.05).Accuracy The specificity and sensitivity of the test group were significantly higher and had statistical differences.In real-time ultrasound elastography diagnosis,there were 109 benign lesions,of which 12 were grade 0,21 were grade I,53 were grade II,17 were grade III,and 6 were grade IV.There were 49 cases of malignant lesions,7 cases were grade II,19 cases were grade III,and 23 cases were grade IV.Conclusion The use of real-time ultrasound elastography in the diagnosis of thyroid space-occupying lesions has a high diagnostic accuracy.The location of lesions in patients with thyroid occupying lesions should be further clarified and diagnosed.It is worth learning from them.
