结核基因芯片技术在结核耐药快速检测中的应用效果分析 被引量:1

摘要 目的分析结核基因芯片技术在结核耐药快速检测中的应用效果。方法选取2016年11月~2017年11月本院感染科收治的126例肺结核患者为研究对象。所有患者随机留取一份5 ml由肺深部咳出的晨痰标本。对痰标本均进行荧光定量分析以及基因芯片技术检测。结果 (1)126例患者的痰标本中,经痰涂片发现阳性痰39例;经荧光定量试验发现阳性标本42例,非结核分枝杆菌两例,耐药标本10例,其中两例耐多药;基因芯片技术未检测到阳性标本40例,非结核分枝杆菌2例,无耐药标本8例,其中2例耐多药;因此可用于有效分析药敏结果 40例;(2)基因芯片技术对涂阳肺结核耐多药患者与荧光定量分析检测结果比较差异无统计学意义;检测耐多药灵敏度为76.92%,特异度为96.27%,Kappa值为0.642,阳性预测值为47.6%,阴性预测值为98.95%。结论基因芯片技术检测结核分枝杆菌耐药性,其检测结果与荧光定量分析试验高度一致,且耗时短,可弥补传统检测方法的不足。 From November 2016 to November 2017,126 patients with pulmonary tuberculosis admitted to the infectious disease department of our hospital were selected as the research subjects.All patients were randomly assigned to receive a morning sputum specimen from 5 ml.The sputum samples were analyzed by fluorescence quantitative analysis and gene chip technology.Results①In the sputum specimens of 126 cases,positive sputum was found in 39 cases by sputum smear,42 cases of positive specimens,two cases of non tuberculous Mycobacterium and 10 drug resistant specimens,two cases of multi drug resistance,40 cases of positive specimens,2 non tuberculous Mycobacterium and 8 non resistant specimens by gene chip technology.Among them,two cases were multi drug resistant,so it could be used for effective analysis of 40 cases of drug sensitivity.②The gene chip technology had no significant difference between the smear positive pulmonary tuberculosis patients with multi drug resistance and the fluorescence quantitative analysis.The sensitivity of the multidrug detection was 76.92%,the specificity was 96.27%,the Kappa value was 0.642,the positive predictive value was 47.6%,and the negative predictive value was 98.95%.Conclusion Gene chip technology was used to detect the drug resistance of Mycobacterium tuberculosis.The detection results were in good agreement with the fluorescence quantitative analysis test,and the time consuming was short,which could make up the shortage of traditional detection methods.
