摘要
目的探究老年双膝关节骨关节炎患者接受同期和分期全膝关节置换术治疗的价值。方法选取2014年1月~2017年1月老年双膝关节骨关节炎患者80例,根据其就诊时间分为对照组和实验组,每组40例。对照组实施分期全膝关节置换术治疗,实验组则实施同期全膝关节置换术治疗,对比两组患者治疗结果。结果实验组患者手术时间、住院时间明显短于对照组(P<0.05);实验组患者输血量、引流量明显高于对照组(P<0.05);两组患者术后6个月的膝关节功能、关节活动度、骨关节炎症状均较治疗前改善(P<0.05),两组之间相比,差异无统计学意义;实验组并发症发生率为12.50%,明显高于对照组并发症发生率(0.00%)(P<0.05)。结论老年双膝关节骨关节炎患者接受同期和分期全膝关节置换术治疗,均可取得一定的价值,其中,分期全膝关节置换术的治疗时间和住院时间较长,但是其输血量较小,能够将患者出现切口感染等并发症的几率降低,在老年患者中应用,具有一定的优势。
Objective To explore the value of the treatment of elderly knee osteoarthritis patients undergoing simultaneous total knee and total knee arthroplasty.Methods Eighty elderly patients with double knee osteoarthritis from January 2014 to January 2017 were selected and divided into a control group and an experimental group according to their visiting time.There were 40 cases in each group.The control group was treated with staged total knee arthroplasty,and the experimental group was treated with total knee arthroplasty at the same time.The results of the two groups were compared.Results The operation time and length of stay in the experimental group were significantly shorter than those in the control group(P<0.05).The blood transfusion volume and drainage volume were significantly higher in the experimental group than in the control group(P<0.05);The function,degree of motion,and osteoarthritis symptoms were improved(P<0.05).There was no significant difference between the two groups.The complication rate in the experimental group was 12.50%,which was significantly higher than that of the control group.Incidence rate(0.00%)(P<0.05).Conclusion Both elderly patients with osteoarthritis of the double knee can receive certain value in the same period and staging total knee arthroplasty.Among them,staging total knee arthroplasty has longer treatment time and length of hospital stay,but its blood transfusion is smaller.It can reduce the incidence of complications such as incision infection in patients,and it has certain advantages in elderly patients.
