摘要 目的探讨Delta内镜技术用于治疗腰椎管狭窄症的临床效果。方法选择2015年1月至2016年6月河南省直第三人民医院收治的108例腰椎管狭窄症患者为研究对象,根据手术方法将患者分为观察组和对照组,每组54例。观察组患者采用Delta内镜技术经后路单侧椎板间入路双侧椎管减压术治疗,对照组患者给予后路椎管减压植骨融合内固定术。记录2组患者的手术时间、术中出血量和住院时间;分别于手术前及术后12个月采用视觉模拟评分法(VAS)评估患者疼痛程度,采用Oswestry功能障碍指数(ODI)问卷表评估患者机体功能;术后12个月,采用改良Mac Nab标准评定2组患者的临床疗效。结果观察组患者的手术时间和住院时间显著短于对照组,术中出血量显著少于对照组(P<0.05)。2组患者术前VAS评分及ODI评分比较差异均无统计学意义(P>0.05);2组患者术后VAS评分及ODI评分显著低于术前(P<0.05),术后观察组患者VAS评分及ODI评分显著低于对照组(P<0.05)。观察组和对照组患者治疗效果优良率分别为92.6%(50/54)、77.8%(42/54),观察组患者治疗效果优良率显著高于对照组(χ2=4.696,P<0.05)。结论 Delta内镜技术治疗腰椎管狭窄症效果显著,可缩短手术时间,减少术中出血量,缓解患者疼痛,改善患者机体功能。 Objective To investigate the effect of Delta endoscopic technique in the treatment of lumbar spinal stenosis.Methods A total of 108 patients with lumbar spinal stenosis in the Third People′s Hospital of Henan Province from January 2015 to June 2016 were selected as the subjects.The patients were divided into observation group and control group according to operation method,with 54 cases in each group.The patients in the observation group were treated with Delta endoscopic hibateral spinal canal decompression through posterior unilateral interlaminar approach,and the patients in the control group were treated with posterior spinal canal decompression and bone graft fusion internal fixation.The operation time,intraoperative bleeding and hospitalization time of the patients in the two groups were recorded.The pain of the patients was evaluated by visual analogue scale(VAS),and the body function of the patients was evaluated by Oswestry disability index(ODI)questionnaire before and 12 months after operation.The clinical effect was evaluated by the modified MacNab standard at 12 months after operation.Results The operation time and hospitalization time in the observation group were significantly shorter than those in the control group,and the intraoperative bleeding in the observation group was significantly less than that in the control group(P<0.05).There was no significant difference in preoperative VAS score and ODI score between the two groups(P>0.05).The VAS score and ODI score after operation were significantly lower than those before operation in the two groups(P<0.05).The VAS score and ODI score in the observation group were significantly lower than those in the control group after operation(P<0.05).The fineness rate in the observation group and the control group was 92.6%(50/54)and 77.8%(42/54)respectively,the fineness rate in the observation group was significantly higher than that in the control group(χ2=4.696,P<0.05).Conclusion The effect of Delta endoscopic technique in the treatment of lumbar spinal
