摘要
目的通过对76例丙型肝炎病毒(HCV)基因分型实验室检测结果并结合临床资料进行分析,探讨HCV基因分型的临床应用价值。方法收集一汽总医院住院患者抗-HCV和丙型肝炎病毒RNA核酸定量均为阳性的标本,采用丙型肝炎基因分型特异性引物及荧光探针,应用一步法聚合酶链式反应(RT-PCR)结合Taqman技术对HCV进行Ⅰ型和非Ⅰ型的荧光PCR分型检测。结果检测76例标本共检出两种基因型,分别为:Ⅰ型基因30例,非Ⅰ型基因46例,Ⅰ型基因占39.5%,非Ⅰ型基因占60.5%;男性中Ⅰ型基因占44.7%,女性中Ⅰ型基因占31.0%;76例感染者有输血者40例,占感染人数的52.6%。结论 76例丙型肝炎病毒基因分型结果 HCV基因型主要为非Ⅰ型,这与中国北方地区HCV基因型分布比较一致。在感染丙型肝炎病毒的人群中,有输入血液制品史的人占52.6%,说明输血是感染丙型肝炎病毒的一个主要途径。76例感染者各基因型男女感染的比例差异无统计学意义,通过输血感染丙型肝炎病毒无性别差异。综合分析,HCV基因分型对丙型肝炎的预防、诊断、治疗、个体化用药以及愈后评估都具有重要意义。
Objective To explore the clinical value of HCV genotyping by analyzing the results of 76 cases of hepatitis C virus(HCV)genotyping laboratory.Methods The samples of anti-HCV and HCV RNA nucleic acid were both positive,and the specific primers and fluorescent probes of hepatitis C genotyping were used to detect HCV and non I type fluorescent PCR typing by one step polymerase chain reaction(RT-PCR)combined with Taqman technique.Results Two genotypes were detected in 76 samples:30(39.5%)for type I,46(60.5%)for non-type I,44.7%in male and 31.0%in women,40(52.6%)infected patients.Conclusion 76 genotypes of HCV genotypes show that the HCV genotype is mainly non type I,which is consistent with the distribution of HCV genotypes in northern China.Among those infected with HCV,52.6%of whom has imported blood products history shows that blood transfusion is a major route of infection with HCV.There is no significant difference in the proportion of male and female infection among 76 infected patients.There is no gender difference in the infection of HCV through blood transfusion.Comprehensive analysis shows that HCV genotyping is of great importance to the prevention,diagnosis,treatment,individualized medication and post evaluation of hepatitis C.
