摘要
目的 整理本院西医门诊患者的用药处方资料,探讨门诊西药房的临床用药过程中所出现的不合理处方,并进行提升对策分析。方法 选取本院2016年11月至2017年11月门诊西药房开具的处方2 568张,对处方中不合理问题进行分析,总结不合理处方的发生因素,并进行对策探讨。结果 不合理处方265张,处方用药问题率10.31%,药物选择不合理20.75%,联用不合理13.96%,剂量过多15.47%,用药次数不合理8.30%,给药方式不合理21.50%,用药溶媒不合理8.30%,用药时间间隔不合理11.69%。结论 西医门诊在使用药物过程中,如果不能按照相关规定与制度的规范严格进行工作,就会出现不合理处方,且药剂师的职业素养与专业能力,也会直接影响门诊用药处方的合理性,应加强对药剂师的职业素养与技能培养,严格门诊西药房用药制度,提高用药处方合理性。
Objective To organize the prescription data of the patients in the western medicine department of our hospital,and to explore the unreasonable prescriptions in the clinical application of western medicine pharmacy at the outpatient department,and to analyze the countermeasures.Methods From November 2016 to November 2017,2 568 prescriptions at the outpatient pharmacy were selected;the unreasonable problems in the prescriptions were analyzed;and the happening of the unreasonable prescription and countermeasures were summarized.Results There were unreasonable 265 prescriptions;10.31%got prescription medication problems,20.75%unreasonable drug selection,13.96%unreasonable combination,15.47%over-dosage,8.30%unreasonable frequency,21.50%unreasonable drug delivery ways,8.30%unreasonable drug solvent,and 11.69%unreasonable interval.Conclusion During the process of drug use at out-patient department,if the work can't be carried out in accordance with the regulations and system specification strictly,unreasonable prescriptions will appear;pharmacists'professional quality and ability will directly affect the rationality of outpatient medicine prescriptions;the professional quality of pharmacists and skill training should strengthened;the system of outpatient pharmacy administration should be strict;and drug prescription rationality should be improved.
