摘要
目的探讨不同盆底康复疗法对再生育妇女产后盆底肌康复的治疗效果。方法选取2016年1月至2017年2月于本院就诊的96例再生育产妇为研究对象,采用简单随机抽样法将其分为A组、B组与C组,每组各32例。在常规康复模式下,A组、B组、C组产妇分别接受盆底康复操、盆底肌电生物反馈治疗及两者的联合治疗。比较干预前后三组产妇盆底Ⅰ类与Ⅱ类肌纤维肌力、阴道肌电电压、夜尿次数、压力性尿失禁(stress urinary incontinence,SUI)和盆腔器官脱垂(pelvic organ prolapse,POP)的发生率。结果干预后3个月和6个月,C组产妇盆底Ⅰ类与Ⅱ类肌纤维肌力为0~Ⅱ级的比例显著降低(P<0.05)。干预后6个月,A组产妇Ⅰ类肌纤维肌力为0~Ⅱ级的产妇比例显著高于C组(P<0.05)。干预后,三组产妇的阴道肌电电压均显著升高(P<0.05),夜尿次数均显著减少,且干预后6个月C组产妇的POP发生率显著低于干预前(P<0.05)。干预后,三组产妇的阴道肌电电压、夜尿次数比较均具有显著差异(P<0.05),其中阴道肌电电压:A组<B组<C组,夜尿次数:A组>B组>C组。结论对于再生育产后妇女而言,盆底康复操联合肌电生物反馈治疗能显著促进其盆底肌的康复,提高阴道肌电电压,减少夜尿次数及POP的发生,值得临床推广应用。
Objective To explore the effects of different pelvic floor rehabilitation therapies on the recovery of postpartum pelvic floor muscle in reproduction females.Method 96 reproduction females admitted in our hospital from January 2016 to February 2017 were selected and they were divided into group A,group B and group C according to simple random sampling,each group has 32 cases.Based on conventional rehabilitation nursing mode,puerperae in group A,group B and group C were given pelvic floor rehabilitation gymnastics,pelvic floor electromyography biofeedback therapy and combination therapy respectively.The pelvic floor muscle strength of typeⅠandⅡmuscle fiber,vaginal electromyographic voltage,nocturnal frequency,incidences of stress urinary incontinence(SUI)and pelvic organ prolapse(POP)were compared among three groups before and 3 months and 6 months after intervention.Result The ratio of 0~Ⅱlevel of typeⅠandⅡmuscle fiber in group C significantly decreased after intervention(P<0.05).For inter-group comparison,the ratio of 0~Ⅱlevel of typeⅠmuscle fiber in group A was significantly higher than that in group C,6 after months(P<0.05).The vaginal electromyographic voltage in three groups significantly increased after intervention,and the nocturnal frequency significantly decreased after intervention,and the incidence of POP in group C significantly decreased 6 months after intervention(P<0.05).There were statistically significant differences in vaginal electromyographic voltage and nocturnal frequency among three groups after intervention(P<0.05).Specifically,vaginal electromyographic voltage:group A<group B<group C;nocturia frequency:group A>group B>group C.Conclusion For reproduction females,pelvic floor rehabilitation gymnastics combined with electromyography biofeedback therapy could significantly promote the rehabilitation of pelvic floor muscles,improve vaginal myoelectric voltage,reduce nocturnal frequency and preventing occurrence of POP,and it is worthy of clinical application.
