摘要
目的:探讨早孕先兆流产患者甲状腺功能异常情况。方法:选择2016年1月-2017年7月于我院接受保胎治疗的60例早孕先兆流产患者作为研究对象。抽取所有患者空腹静脉血,检测血清促甲状腺激素(TSH)、游离甲状腺素(FT4)、甲状腺过氧化物酶抗体(TPO-Ab)。比较早孕先兆流产构成与甲状腺功能异常情况,记录所有患者TSH、FT4、TPO-Ab指标平均水平。结果:甲功正常45.00%、甲减51.67%、甲亢3.33%(2/60);与复发性自然流产史者相比,不良孕史、初孕先兆流产者甲功正常概率更高,差异有统计学意义(P<0.05);与复发性自然流产史者相比,不良孕史、初孕先兆流产者甲减发生概率更低,差异有统计学意义(P<0.05);60例患者TSH(5.31±3.69)m IU/L、FT4(13.06±4.18)pmol/L、TPO-Ab(69.86±15.79)U/m L。结论:早孕先兆流产患者以合并甲减为主,对早孕先兆流产患者血TSH、FT4、TPO-Ab变化情况进行监测可有效辅助临床诊治。
Objective:To explore the relationship between threatened abortion and abnormal thyroid function in early pregnancy.Methods:60 cases of early pregnancy threatened abortion treated in our hospital from January 2016 to July 2017 were selected as the study subjects.All patients were selected for detecting fasting venous blood,and serum thyrotropin(TSH),free thyroxine(FT4),and thyroid peroxidase antibody(tpo-ab).Compared with the abnormal condition of thyroid function,the average level of TSH,FT4 and tpo-ab in all patients was recorded.Results:The normal thyroid function proportion was 45.00%,hypothyroidism was 51.67%and hyperthyreosis was 3.33%(2/60).Compared with those with recurrent spontaneous abortion,there was a higher probability of the normal pregnancy of the pregnant women with the preconception of pregnancy,and the difference was statistically significant(P<0.05).Compared with those with recurrent spontaneous abortion,there was a lower incidence of the occurrence of the first trimester in the history of adverse pregnancy and early pregnancy,and the difference was statistically significant(P<0.05).In 60 patients,TSH was(5.31±3.69)mIU/L,FT4 was(13.06±4.18)pmol/L,and tpo-ab was(69.86±15.79)U/ml.Conclusion:The changes of TSH,FT4 and tpo-ab in patients with early pregnancy threatened abortion were effectively assisted in clinical diagnosis and treatment.
