摘要
目的探讨经皮椎体成形术对骨质疏松性椎体压缩骨折患者全脊柱矢状位平衡的影响。方法选取2013年5月～2017年5月期间我院骨科接收的130例单节段骨质疏松椎体压缩骨折患者设为观察组,同时选取同期来本院体检的健康老年人60例作为对照组,观察组采取PVP治疗,对比两组脊柱-骨盆矢状位参数,并观察PVP治疗效果。结果术前与对照组比较,观察组C7-S1SVA显著要长,TK、PT显著增大,LL、SS显著减小,差异均有统计学意义(P<0.05);术后6个月时,患者椎体中柱高度显著提高,Cobb角及Oswestry评分、VAS评分显著均明显降低(P均<0.05);术后患者C7-S1SVA明显减小,LL明显增大,TK明显减少,差异均有统计学意义(P均<0.05)。结论骨质疏松性椎体压缩骨折患者与健康老年人相比存在明显的前矢状位不平衡,采取PVP治疗能改善脊柱矢状位平衡状态,改善患者功能,具有推广价值。
Objective To investigate the effect of percutaneous vertebroplasty on the spinal sagittal balance of patients with osteoporotic vertebral compression fractures.Methods 130 patients with single-segment osteoporotic vertebral compression fractures were selected from the department of orthopaedics in our hospital from May 2013 to May 2017.The patients were set as the observation group.At the same time,60 healthy elderly patients who underwent physical examination in our hospital were selected as the control group.The observation group was treated with PVP.The spine-pelvis sagittal parameters were compared between the two groups,and the effect of PVP treatment was observed.Results Compared with that of the control group,C7-S1SVA in the observation group was significantly longer,TK and PT were significantly increased,and LL and SS were significantly decreased before operation,and the difference was significant(P<0.05).The height of the vertebral middle Column increased significantly,and Cobb's angle,Oswestry's score and VAS score significantly decreased at 6 months after operation(all P<0.05).After surgery,the C7-S1SVA decreased significantly,LL increased significantly,and TK decreased significantly in patients,all with statistically significant(P<0.05).Conclusion Patients with osteoporotic vertebral compression fractures have obvious anterior sagittal imbalance compared with healthy elderly patients.PVP treatment can improve sagittal balance of spine and improve patient function,which has promotion value.
