某院2013-2016年尿培养中的病原菌分布及耐药性分析

摘要 目的 探讨医院尿培养分离出病原菌的分布特点及其耐药性,指导临床合理运用抗菌药物,为泌尿系感染的诊断和治疗提供依据。方法 收集该院2013年至2016年合格的尿培养标本,采用MicroScan Walkaway96系统对分离病原菌进行鉴定及药敏试验,应用WHONET5.6软件对数据进行统计分析。结果 2013-2016年共送检15 658例尿培养标本,分离出6 660株病原菌,阳性率为42.53%。剔除同一患者所获重复株分离的菌株4 455例病原菌,其中革兰阴性菌2 821株,占63.30%,尤以大肠埃希菌最为常见;革兰阳性菌1 266株,占28.40%,以葡萄球菌属和肠球菌属为主;真菌364株,占8.17%。病原菌对各种抗菌药物耐药差异性较大,表现为多药耐药。结论 该院尿路感染病原菌种类复杂多样、耐药程度严重,需及时监测病原菌的菌群种类分布和耐药变迁,以指导临床合理、规范地使用抗菌药物,提高尿路感染的治疗质量。 Objective To investigate the distribution characteristics of pathogenic bacteria isolated from urine specimens of hospital and its drug resistance,so as to provide a reference for rational use of antibiotics in clinical practice.Methods Collect all the data of the qualified urine culture specimens in the hospital from 2013 to 2016.And the identification and drug-sensitivity of the bacteria were tested by the system MicroScan Walkaway 96 and the statistical analysis of data was finished by the software WHONET 5.6.Results There were 15 658 urine culture specimens in total from 2013 to 2016,of which 6 660 strains were isolated,with a positive rate of 42.53%.And 4 455 strains were isolated after eliminating the repeat bacterial strains of the same patient.Among them,2 821 strains were gram negative bacteria,accounting for 63.30%,especially Escherichia coli.is the most common;1 266 strains were gram positive bacteria,accounting for 28.40%,mainly Staphylococcus and enterococcus;364 strains were fungi,accounting for 8.17%.Conclusion The pathogenic bacteria of urinary tract infection in the hospital are complex and diverse,and the degree of drug resistance is serious.So it′s necessary to monitor the species distribution and drug resistance of pathogens timely,so as to guide the rational and standardized use of antibiotics and improve the quality of treatment of urinary tract infections.
