摘要
目的探讨亚甲蓝示踪剂前哨淋巴结活检术(SLNB)在早期乳腺癌治疗中的临床应用可行性。方法回顾性分析该院2015年10月—2016年10月收治的129例T1或T2早期乳腺癌患者的前哨淋巴结活检术(SLNB)的病例资料,同时行腋窝淋巴结清扫术(ALND)作为对照组的研究对象,分析前哨淋巴结活检结果及亚甲蓝法染色法SLNB术对乳腺癌SLN检出率的影响因素。结果观察129患者术后前哨淋巴结及腋窝淋巴结的病理结果,在120例患者中检出前哨淋巴结(SLN),检出率为93.0%,敏感度为91.9%,腋窝淋巴结检测假阴性率为8.10%。肿瘤大小、BIM、术前化疗均是亚甲蓝法染色法SLNB术对乳腺癌SLN检出率的影响因素。结论前哨淋巴结活检术(SLNB)安全可靠、基层易于推广,能准确预测早期乳腺癌腋窝淋巴结分期,减轻腋窝淋巴结清扫(ALND),为基层医院患者实施保腋窝的乳腺癌改良根治术提供临床支持。
Objective This paper tries to investigate the clinical application of methylene blue tracer sentinel lymph node biopsy(SLNB)in the treatment of early stage breast cancer.Methods A retrospective analysis of this hospital from October 2015 to October 2016 in 129 cases of T1 or T2 early breast cancer patients with sentinel lymph node biopsy(SLNB)of the case data,and axillary lymph node dissection(ALND)as the control group of subjects,the influencing factors of sentinel lymph node biopsy results and methylene blue staining SLNB detection rate of breast cancer SLN were analyzed.Results Sentinel lymph nodes and axillary lymph nodes in 129 patients were observed pathologically.Sentinel lymph node(SLN)was detected in 120 patients.The detection rate was 93.0%,the sensitivity was 91.9%,and the false negative rate of axillary lymph node was 8.10%.Tumor size,BIM,and preoperative chemotherapy were all the factors influencing the detection rate of SLN in breast cancer by methylene blue staining.Conclusion Sentinel lymph node biopsy(SLNB)is safe and reliable,and is easy to spread at the grass-roots level.It accurately predicts axillary lymph node staging and axillary lymph node dissection(ALND)in early-stage breast cancer and provides clinical support for modified radical mastectomy for axillary breast cancer in primary hospital patients.
