摘要
目的探讨在宫缩乏力性产后出血预防中应用卡贝缩宫素的临床价值,分析其对产妇产后出血量、心率、血红蛋白的影响。方法方便选取2015年5月—2017年5月期间该院妇产科收治的70例足月妊娠阴道分娩后宫缩乏力患者纳入该次研究,按照不同缩宫素给予情况将其分成甲组(n=35)和乙组(n=35),对两组娩出胎儿患者分别给予缩宫素和卡贝缩宫素,观察其产后出血发生情况、心率变化情况及血红蛋白变化情况。结果甲组宫缩乏力患者接受药物干预后的产后30 min出血量[(160.5±20.5)mL]、产后2 h出血量[(235.6±25.3)mL]、产后24 h出血量[(350.4±36.1)mL]及产后24 h血红蛋白下降值[(13.6±5.7)g/L]与乙组[(92.3±11.8)mL、(144.2±31.3)mL、(200.2±37.6)mL、(20.3±7.1)g/L]相比,差异有统计学意义(t=17.057 7、13.435 4、17.047 5、4.353 4,P<0.05);而前者接受药物干预后心率变化情况与乙组相比,差异无统计学意义(P>0.05)。结论在宫缩乏力性产后出血预防中应用卡贝缩宫素的临床价值较高,能够安全、快速、有效的止血。
Objective This paper tries to investigate the clinical value of carbene oxytocin in the prevention of postpartum hemorrhage in uterine inertia,and to analyze its effect on maternal postpartum hemorrhage,heart rate and hemoglobin.Methods From May 2015 to May 2017,70 cases of obstetrics and gynecology in the hospital were convenient selected and included in this study.According to the different oxytocin administration,the patients were divided into group A(n=35)and group B(n=35).The oxytocin and carboin were given to the fetus in the two groups.The incidence of postpartum hemorrhage,heart rate change and hemoglobin were observed.Results A group of uterine inertia patients after receiving drug intervention were 30min postpartum hemorrhage[(160.5±20.5)mL],2 h postpartum hemorrhage[(235.6±25.3)mL],and 24 h postpartum hemorrhage[(350.4±36.1)mL]and the hemoglobin decrease in postpartum group[(13.6±5.7)g/L]was significantly higher than that in group B[(92.3±11.8)mL,(144.2±31.3)mL,(200.2±37.6)mL and(20.3±7.1)g/L],the difference was statistically significant(t=17.057 7,13.435 4,17.047 5,4.353 4,P<0.05).While the former after receiving drug intervention heart rate changed and Compared with the group B,the difference was not statistically significant(P>0.05).Conclusion The clinical value of carbene oxytocin in the prevention of postpartum hemorrhage in uterine inertia is high,which can safely,quickly and effectively stop bleeding.
