摘要
目的探讨突发性耳聋患者内耳循环障碍与血脂水平的相关性。方法选择我院诊断及治疗的突发性耳聋患者86例为突聋组,同时选取同一时期来我院体检的健康者102名为健康组,2组均进行内耳道磁共振成像(MRI)扫描和血清总胆固醇(TC)、甘油三酯(TG)检测。结果与突聋组比较,健康组内听动脉、小脑前下动脉、基底动脉指标较高,TC、TG水平较低(P<0.05)。健康组在内听动脉、小脑前下动脉、基底动脉上,高于低频型组、高频型组、平坦型组、全聋型组,在TC、TG水平上低于低频型组、高频型组、平坦型组、全聋型组(P<0.05)。突聋患者不同严重程度患者间内耳微循环与血脂水平差异有统计学意义(P<0.05),在内听动脉、小脑前下动脉、基底动脉上,健康组>轻度组、中度组>重度组>极重度组,在TC、TG水平上,健康组<轻度组<中度组<重度组<极重度组,差异有统计学意义(P<0.05)。突聋患者内耳微循环指标与血脂水平呈负相关(P<0.05)。结论突发性耳聋患者存在内耳循环障碍和血脂异常,且二者之间呈负相关,对突发性耳聋的诊治具有一定的参考价值。
Objective To investigate the correlation between inner ear circulatory disorder and blood lipid level in patients with sudden hearing loss.Methods Eighty-six cases of sudden hearing loss were selected as the sudden deafness group,and 102 cases of healthy volunteers were selected as the normal group.Inner ear were detected by MRI and serum total cholesterol(TC)and triglyceride(TG)were also tested in the 2 groups.Results Compared with the sudden deafness group,indexes of internal auditory artery,anterior inferior cerebellar artery and basilar artery in the normal group were higher,and levels of TC and TG were lower(P<0.05).Indexes of internal auditory artery,anterior inferior cerebellar artery and basilar artery in the normal group were higher than those of the low frequency group,high frequency group,flat group,complete deafness group,and levels of TC,TG in the normal group were lower than those of the low frequency group,high frequency group,flat group,complete deafness group(P<0.05).There were statistical differences in the level of inner ear microcirculation and blood lipid among patients with different severity in sudden deafness(P<0.05).Indexes of internal auditory artery,the anterior inferior cerebellar artery,and the basilar artery in the normal group was higher than those of the mild group,moderate group,severe group and extreme severe group.Levels of TC,TG in the normal group was lower than those of the mild group,moderate group,severe group and extreme severe group(P<0.05).There was a significant negative correlation between microcirculation index and serum lipid level in patients with sudden deafness(P<0.05).Conclusion There is a negative correlation between the inner ear circulation disorder and dyslipidemia in patients with sudden deafness,and the diagnosis and treatment of sudden deafness is of certain reference value.
