摘要 目的:分析氨甲环酸在非体外循环冠脉搭桥术中的应用效果。方法:抽取2014年7月-2017年7月前来我院接受非体外循环冠脉搭桥术的64例患者,随机将其分为研究组和对照组各32例。研究组患者术中切片时静脉滴注氨甲环酸与5%葡萄糖注射液,对照组患者静脉滴注抑肽酶与5%葡萄糖混合液,观察两组患者手术时间、术中失血量、术毕引流量、输血人数、手术前后血小板变化情况及不良反应发生情况。结果:研究组手术时间短于对照组,术中失血量、术毕引流量及输血人数均少于对照组,组间数据差异较大,有统计学意义(P<0.05);术后两组患者均未出现严重不良反应,患者血小板数量均有改善,术后不同时段研究组患者血小板数量均高于对照组,但组间数据差异不大,无统计学意义(P>0.05)。结论:接受非体外循环冠脉搭桥术的患者术中静脉滴注氨甲环酸较之抑肽酶,疗效更佳,不仅可抑制纤溶酶、止血、消炎,还能抗变态反应,可减少术中和术后出血量,同时不会增加血栓栓塞时间发生率,此次显示,患者术中静脉滴注氨甲环酸,能有效缩短手术时间、减少术中出血量、术毕引流量及输血人数,同时可改善患者手术前后血小板变化情况,减少不良反应,疗效显著,提高了用药安全性,值得临床推广应用。 Objective:To analyze the application effects of carbamicylic acid on off-pump coronary artery bypass grafting.Methods:A total of 64 cases of off-pump coronary artery bypass grafting patients treated in our hospital from July 2014 to July 2017 were selected and randomly divided into the research group and the control group,with 32 cases in each group.During intraoperative section,the patients in the research group received intravenous injection of carbamicylic acid and 5%glucose injection;while the control group received intravenous injection of Aprotinin and 5%glucose mixture.The time of operation,the amount of blood loss during operation,the drainage flow,the number of blood transfusion,the changes of platelets before and after operation and the occurrence of adverse reactions were observed in two groups.Results:The time of operation in the research group was shorter than that in the control group;the amount of blood lost during operation,the volume of blood drainage and the number of blood transfusion in the research group were lower than those in the control group,and there was significant difference between two groups(P<0.05).There was no serious adverse reaction in two groups after operation.The number of platelets in the research group was higher than that in the control group at different time after operation,but there was no significant difference between the two groups(P>0.05).Conclusion:Carbamicylic acid had a better effect on patients underwent off-pump coronary artery bypass grafting;it could not only inhibit fibrinolytic enzyme,hemostatic,anti-inflammatory,but also resist allergic reaction,reduce the amount of blood loss during and after operation,and did not increase the incidence of thromboembolism time.The research found that intraoperative intravenous instillation of carbamicylic acid could effectively shorten the operation time,reduce the amount of intraoperative blood loss,postoperative drainage and blood transfusion,at the same time it could improve the change of platelet before and after oper
