摘要
目的运用纤维束追踪的空间统计学技术(tract-based spatial statistics,TBSS)探讨梗死后抑郁症(post-stroke depression,PSD)(和3个对照组受试者成组比较)患者的脑白质微观结构的损害。材料与方法对28例梗死后抑郁症患者组和18例梗死后无抑郁症患者组、13例单纯抑郁症患者组、11例正常对照受试者组行全脑扩散张量成像扫描。应用TBSS方法对PSD组和3个对照组的部分各向异性(fractional anisotropy,FA)值进行组间比较。结果与3个对照组相比,PSD组左侧内囊后肢、胼胝体压部的FA值显著降低(P<0.05)。两两组间比较,左侧内囊后肢主要为PSD组与正常对照组间差异有统计学意义(P<0.05);胼胝体压部主要为PSD组与正常对照组、PSD组与单纯抑郁症组间差异有统计学意义(P<0.05)。结论 TBSS技术作为可信度较高的最新扩散张量成像研究方法,可以发现早期PSD即存在与调节情绪和认知关系联络神经纤维及脑白质的损伤。
Objective:To investigate the microstructural abnormalities of white matter in patients with post-stroke depression(PSD)by using the method of tract based spatial statistics(TBSS).Materials and Methods:Whole brain diffusion tensor imaging(DTI)was performed in 28 patients with PSD and 3 control groups,including 18 stroke patients without depression group(PSND),13 patients with depression group(De)and 11 healthy volunteers(NC).Fractional anisotropy(FA)values were compared between the PSD group and 3 control groups by TBSS.Results:Compared with the 3 control groups,FA values of white matter in the left posterior limb of internal capsule and the splenium of corpus callosum were significantly decreased in the patients with PSD(P<0.05).Inter-group differences with statistical significance were found at the left posterior limb of internal capsule in following:PSD vs NC group(P<0.05);at the splenium of corpus callosum in following:PSD vs NC group(P<0.05),PSD vs De group(P<0.05).Conclusions:As the latest and higher credibility DTI research method,TBSS technology could be used to find the microstructural abnormalities of white matter integrity and disruption of neural connections among regions regulating mood and cognition in patients with early PSD.
