摘要
目的探讨社区护理干预对老年高血压患者的效果及对心血管事件的风险控制疗效。方法 96例老年原发性高血压患者,按照随机数字表法分为对照组和观察组,每组48例。对照组给予常规用药指导,观察组在对照组的基础上给予社区护理干预。比较两组患者干预前、干预3个月后血压、依从性;随访1年,比较两组患者高血压所致心血管不良事件发生情况。结果干预3个月后,两组患者收缩压、舒张压均较干预前降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。对照组患者干预前与干预3个月后依从性比较差异无统计学意义(P>0.05);观察组干预3个月后依从性明显优于本组干预前,且优于对照组干预3个月后,差异具有统计学意义(P<0.05)。随访1年,观察组心血管事件发生率为12.50%,明显低于对照组的37.50%,差异具有统计学意义(P<0.05)。结论社区护理干预应用于老年高血压患者中可明显改善血压,提高依从性,减少心血管事件的发生,效果优于单纯药物指导。
Objective To discuss the effect of community nursing intervention on elderly patients with hypertension and the risk control efficacy on cardiovascular events.Methods A total of 96 elderly patients with primary hypertension were divided by random number table method into control group and observation group,with 48 cases in each group.The control group received routine medication guidance,and the observation group received community nursing intervention on the basis of the control group.Blood pressure and compliance were compared in the two groups before intervention and after 3 months of intervention,and adverse cardiovascular events caused by hypertension in two groups was compared after 1 year of follow-up.Results After 3 months of intervention,both groups had lower systolic blood pressure and diastolic blood pressure than before intervention,and the observation group was lower than the control group.Their difference was statistically significant(P<0.05).The control group had no statistically significant difference in compliance before and after 3 months of intervention intervention(P>0.05).The observation group had obviously better compliance after 3 months of intervention intervention than before intervention,and it was better than the control group.Their difference was statistically significant(P<0.05).After 1 year of follow-up,the observation group had obviously lower incidence of adverse cardiovascular events as 12.50%than 37.50%in the control group,and the difference was statistically significant(P<0.05).Conclusion Community nursing intervention can obviously improve blood pressure of elderly patients with hypertension,improve their compliance and reduce the occurrence of cardiovascular events.The effect of community nursing intervention is superior to that of simple drug guidance.
