摘要 目的:利用声脉冲辐射力成像-声触诊组织定量(acoustic radiation force impulse-virtual touch quantification,ARFI-VTQ)技术测定乳腺US-BI-RADS 1类硬度的影响因素探讨。方法:选取40名经二维超声诊断为乳腺US-BI-RADS1类的女性,共计80个乳腺,应用ARFI-VTQ技术于正常女性生理周期的3个不同时期(卵泡期、排卵期、黄体期)分别对两侧乳腺进行剪切波速度值(Vs)测量并进行比较,同时分析受检者年龄、取样深度、乳腺腺体厚度、乳腺侧别对Vs值的影响。结果:单因素结果显示左右两侧乳腺各时期Vs值差异无统计学意义(P>0.05);30岁及以上卵泡期Vs值低于30岁以下,差异有统计学意义(P<0.05);左侧乳腺Vs值与年龄呈负相关、与腺体厚度呈正相关(P<0.05);右侧乳腺Vs值与年龄和测量深度呈负相关(P<0.05);排卵期Vs值与年龄和测量深度呈负相关关系,差异有统计学意义(P<0.05);卵泡期Vs值与年龄呈负相关,差异有统计学意义(P<0.05)。多重线性回归分析结果显示Vs值与年龄段、时期、测量深度有关,与左右侧和腺体厚度无关,30岁及以上Vs值低于30岁以下,排卵期和卵泡期Vs值低于黄体期;测量深度越深,Vs值越低。结论:ARFI-VTQ技术测量乳腺US-BI-RADS 1类硬度与年龄段、生理时期、测量深度等因素相关。 Objective:To explore the factors influencing the stiffness of ultrasound breast imaging-reporting and data system(US-BI-RADS)category 1 measured by the acoustic radiation force impulse-virtual touch quantification(ARFI-VTQ)technique.Methods:A total of 80 breasts from 40 women with US-BI-RADS category 1 findings on two-dimensional ultrasonography were used in this study.With the ARFI-VTQ technique,the shear wave velocity(Vs)values of both breasts were measured and compared during the follicular phase,ovulatory phase,and luteal phase of the normal menstrual cycle.The influence of age,sampling depth,thickness of the mammary gland,and breast side on Vs was also analyzed.Results:The results of one-way analysis of variance showed that there was no significant difference in Vs between the left and right breasts at each phase(P>0.05).The Vs value during the follicular phase was significantly lower in women aged 30 years and above than in those aged under 30 years(P<0.05).The Vs value of the left breast was negatively correlated with age but positively correlated with thickness of the mammary gland(P<0.05).The Vs value of the right breast was negatively correlated with age and measurement depth(P<0.05).The Vs value during the ovulatory phase was negatively correlated with age and measurement depth(P<0.05).The Vs value during the follicular phase was negatively correlated with age(P<0.05).Multiple linear regression analysis demonstrated that the Vs value was associated with age,the phase of the menstrual cycle,and measurement depth,but not with the breast side and thickness of the mammary gland.The Vs value was lower in women aged 30 years and above than in those aged under 30 years,and the Vs value duning the ovulation phase and follicular phase was lower than that of luteal phase.The deeper the measurement depth,the lower the Vs valus.Conclusion:The stiffness of US-BI-RADS category 1 measured by the ARFI-VTQ technique is related to age,the phase of the menstrual cycle,and measurement depth.
