摘要
目的比较FOLFOX6方案与FOLFIRI方案治疗晚期结直肠癌的疗效并探讨肿瘤转移的特点。方法选取晚期结直肠癌患者84例,接受FOLFOX6方案者44例为A组,接受FOLFIRI方案者40例为B组。比较两组患者的有效率、疾病控制率及毒副反应发生情况;比较不同肿瘤发生部位的化疗效果;及晚期结直肠癌的肿瘤转移情况。结果两组有效率、疾病控制率比较,差异均无统计学意义(均P>0.05);直肠癌的疾病控制率高于右半结肠癌;左半结肠癌的有效率高于右半结肠癌(均P<0.05);A组中性粒细胞减少及腹泻的发生率均显著低于B组,而周围神经病变的发生率显著高于B组(均P<0.05)。结肠癌肺转移、骨转移发生率均显著低于直肠癌(均P<0.05)。结论 FOLFOX6方案与FOLFIRI方案治疗晚期结直肠癌均有良好的近期疗效,且左半结肠癌疗效优于右半结肠癌。肝脏、肺脏和骨是晚期结直肠癌常见的转移部位,且直肠癌肺转移、骨转移的发生率更高。
To compare the efficacy and safety of therapeutic regimens of FOLFOX6 and FOLFIRI in the treatment of advanced colorectal cancer and investigate the clinical features of tumor metastasis.Eighty-four patients with advanced colorectal cancer were selected,in which 44 patients received FOLFOX6 therapy were taken as group A,and 40 patients received FOLFIRI therapy were taken as group B.The RR and DCR were compared between the two groups.The efficacy of chemotherapy in different tumor sites and the side effects were also compared between the two groups.Further,tumor metastasis in advanced colorectal cancer was analyzed.There was no significant difference in the RR and DCR between the two groups(P>0.05).However,the DCR of rectal cancer was significantly higher than that of right colon cancer(P<0.05)and the RR of left colon cancer was significantly higher than that of right colon cancer(P<0.05).The incidence of neutropenia and diarrhea was significantly lower while the incidence of peripheral neuropathy was significantly higher in the group A than those in the group B(all P<0.05).The incidence of lung metastasis and bone metastasis of colon cancer was significantly lower than those of rectal cancer(all P<0.05).The therapies of FOLFOX6 and FOLFIRI in the treatment of advanced colorectal cancer all have good short-term efficacy,and the efficacy of left colon cancer is better than right colon cancer.Liver,lung and bone are the most common sites of metastasis in advanced colorectal cancer while the incidence of lung metastasis and bone metastasis was the higher in rectal cancer.
