摘要
目的探讨结缔组织相关性肺间质病(CTD-ILD)的临床特点,分析患者肺部CT表现与肺功能相关性,为指导临床医生对病情的判断及预后提供参考。方法回顾分析我院137例确诊为结缔组织相关性肺间质病的患者,并对胸部CT影像表现、肺功能的结果做相关性分析。结果 137例患者胸部CT检查结果:(1)胸膜下线影96例。(2)小叶间隔增厚64例。(3)不规则纤维索条影119例。(4)磨玻璃影91例。(5)牵拉性支气管扩张21例。(6)蜂窝样灶39例。肺功能结果:137例患者中,5例患者肺功能正常,余患者均为限制性通气功能障碍。轻度肺功能损伤患者58例,中度肺功能损伤43例,重度肺功能损伤31例。经相关性分析,CT评分结果与肺功能DLco、FEV_1、FVC存在负相关性。结论结缔组织相关性肺间质病CT的表现与肺功能有相关性。CT可以在一定程度上反应肺功能损伤的程度。
Objective To investigate the clinical features of connective tissue disease-associated pulmonary interstitial lung disease and the correlation between pulmonary function and CT signs for the clinician to judge the condition and prognosis.Methods 137 patients with connective tissue disease-associated pulmonary interstitial lung disease were analyzed retrospectively.Correlation analysis was performed on the results of pulmonary function in CT images.Results The abnormal changes of CT showed that subpleural shadow in 96 cases,interlobular septal thickening in 64 cases,irregular fibrous streaks in 119 cases,ground glass opacity in 91 cases,traction bronchiectasis in 21 cases,and honeycomb like lesions in 39 cases.The results of CT images were negatively correlated with pulmonary function(DLco,FEV 1,FVC).Conclusion The correlation between CT and pulmonary function tests in CTD-ILD can reflect the degree of lung injury at some extent.
