摘要
目的观察活血促愈汤联合不同方法对多发性肋骨骨折的治疗效果。方法回顾性选取2012年3月至2015年12月就诊的多发性肋骨骨折患者192例,按照治疗方法不同分为A、B、C三组,每组64例。所有患者给予活血促愈汤剂治疗,在此基础上,A组给予纯钛抓握式接骨板治疗,B组给予可吸收肋骨钉治疗,C组仅给予保守方法治疗。观察三组患者总住院时间、胸管留置时间、机械通气时间、视觉模拟评分(VAS)、肺功能指标[6 min步行试验(6MWT)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、最大呼气中段流量(MMEF)、最大呼气流量(PEF)]、治疗有效率、并发症发生率的差异。结果三组患者治疗的有效率无显著差异(P>0.05);B组患者住院时间、胸管留置时间和机械通气时间均较A组和C组短,治疗后VAS评分较A组和C组低;三组患者治疗前肺功能指标无显著差异(P>0.05);治疗后,B组患者6MWT、FVC、FEV1、MMEF、PEF等肺功能指标高于A组和C组;B组患者肺部感染、肺不张、胸廓畸形愈合和骨折不愈合发生率低于A组和C组。结论活血促愈汤联合可吸收肋骨钉对多发性肋骨骨折有较好的治疗效果,且并发症发生率低,具有临床应用价值。
Objective To observe the therapeutic effect of Huoxue promoting soup combined with different methods on multiple ribs fracture.Methods 192 cases of multiple rib fractures from March 2012 to December 2015 were randomly divided into three groups of A,B and C,with 64 cases in each group.All patients were treated by Huoxue promoting soup.On this basis,group A was treated with pure titanium gripping plate,group B was treated with absorbable rib nails,and group C was treated by conservative.The total length of hospital stay,chest tube indwelling time,mechanical ventilation time,visual analogue scale(VAS),pulmonary function parameters(6MWT,FVC and 1 second forced expiratory volume FEV1),maximum expiratory flow(MMEF),maximum expiratory flow(PEF)],treatment effectiveness,and complication rates were observed in the three groups.Results The effective rate of the three groups had no significant difference(P>0.05).Tube indwelling time,mechanical ventilation time and hospitalization time in group B were shorter than those in A group and C group.VAS score in B group after treatment was lower than those of A group and C group;There was no difference in pulmonary function between the three groups before treatment.After treatment,pulmonary function indexes of 6MWT,FVC,FEV1,MMEF,PEF in group B were higher than those in group A and C.The incidence of pulmonary infection,atelectasis,chest malunion and nonunion in B group were lower than those in A group and C group.Conclusion Huoxue promoting soup combined with absorbable rib nail has good therapeutic effect for the treatment of multiple rib fractures,and has low complication rate,so it has clinical application value.
