摘要
目的调查成都地区2013~2015年非瓣膜性心房颤动(房颤)伴高血压患者1年后卒中发生情况,评估卒中发生的危险因素。方法随访1年,根据研究对象是否发生卒中分为卒中组和非卒中组,应用多因素logistic回归模型分析卒中发生的危险因素。结果入选495例房颤伴高血压患者,CHA2DS2-VASc≥2分466例,74例启用了抗凝治疗,抗凝治疗率14.9%,发生缺血性卒中45例,卒中发生率9.1%,多因素Logistic回归分析显示:在房颤伴高血压患者中年龄、女性、吸烟是1年缺血性卒中发生的独立危险因素,抗凝治疗是其保护因素。结论成都地区房颤伴高血压患者1年缺血性卒中发生率较高,抗凝治疗率低,抗凝治疗能降低缺血性卒中发生,年龄、女性、吸烟是缺血性卒中发生的危险因素。
GUO Fu-qiang To investigate the one-year stroke event situation in patients with non-valvular atrial fibrillation associated with hypertension in Chengdu area during the period between 2013 and 2015 and explore the risk factors.After one-year follow-up,the patients were divided into stroke group and non-stroke group according to whether stroke occurred or not.The risk factors of stroke were analyzed by using multivariate logistic regression model.We selected 495 cases of non-valvular atrial fibrillation with hypertension.Of the patients,466 had CHA 2 DS 2-VASc≥2,in which 74 were treated with anticoagulant therapy with 14.9%of anticoagulant treatment rate.There were 45 cases of ischemic stroke with 9.1%of incidence of stroke.Multivariate logistic regression analysis showed that age,female and smoking were the risk factors of ischemic stroke,and anticoagulant therapy was the protective factor for the patients.The incidence of ischemic stroke of patients with atrial fibrillation accompanied by hypertension is higher within 1 year while the rate of anticoagulation therapy is low.Anticoagulant therapy can reduce the incidence of ischemic stroke,and age,female and smoking were the risk factors for ischemic stroke.
