骨导助听器在先天性中外耳畸形患者的应用 被引量:3

摘要 目的观察骨导助听器对先天性中外耳畸形患者的听力改善效果,探索其对该类患者的适应症。方法比较23例先天性中外耳畸形患者(其中12例双侧和11例单侧)佩戴骨导助听器前后的日常交流情况及声场下0.5k-4k Hz纯音测听或条件反射测听平均气导阈值,及日常交流情况。结果平均气导阈值改善值:12例双耳畸形者为31.1d B(助听前63.6d B/后32.5d B);11例单耳畸形者在对侧无掩蔽时为49.9d B(助听前72.3d B/后22.4d B)、对侧有掩蔽时为34.7d B(助听前72.3d B/后37.6d B),对侧有、无掩蔽相差15.2 d B。骨导助听器助听后双侧畸形者可完成日常交流,单侧畸形者声源定位和平衡感改善。结论骨导助听器对先天性中外耳畸形患者,无论是双侧还是单侧畸形都是一种有效、简单、无创、相对经济的听力解决方案。 Objective To explore indications of the Bone conduction hearing aids(BCHA)to patients with congenital deformation of middle and outer ears(CDMOE).Method Compare average air conduction threshold(AACT)of pure tone test or conditioned reflex test in sound field of 23 patients with CDMOE(12 cases of bilateral and 11 unilateral deformity)before and after hearing aid by BCHA at 0.5k-4kHz,and daily communication.Results 23 cases improved obviously in daily communication and the AACT is improved 31.1dB(63.6 dB to 32.5 dB)for 12 bilateral patients,as for 11 unilateral patients it is 49.9 dB(72.3 dB to 22.4dB)without contrallateral masking and 34.7dB(72.3 dB to 37.6 dB)with contrallateral masking,there was 15.2 dB difference between them.Conclution BCHA is a simple,effective,noninvasive,relative economic hearing solution for congenital deformation of middle and outer ears whether bilateral or unilateral deformity.
