三维斑点追踪成像技术评价早期不同构型高血压病患者左室局部收缩功能

Assessment of Left Ventricular Regional Systolic Function by Three-dimensional Speckle Tracking Imaging Echocardiography in Early Stage of Different Patterns of the Left Ventricle of Heart in Hypertensive Patients

摘要 目的应用三维斑点追踪成像(3D-STI)技术评价早期不同构型高血压病(HT)患者左室局部收缩功能的改变。方法对30例左室构型正常的HT患者(构型正常组)、30例左室向心性肥厚的HT患者(向心肥厚组)及30例健康者(对照组)进行3D-STI分析,分别测量三组左室纵向、径向、圆周向及三维应变。结果与对照组比较,构型正常组和向心肥厚组收缩压(SBP)、舒张压(DBP)、左房前后径(LAD)、升主动脉内径(AO)、左室后壁舒张末期厚度(LVPWd)、左心室质量指数(LVMI)增大、E/A值减低(P<0.01);与构型正常组比较,向心肥厚组LAD、室间隔舒张末期厚度(IVSd)、LVPWd、LVMI、相对室壁厚度(RWT)增大,E/A值减低(P<0.01)。与对照组比较,左室构型正常组和向心肥厚组左室纵向收缩期峰值应变值差异有统计学意义的节段集中在左室中间段;左室后间隔基底段和中间段的纵向、径向、圆周向及三维收缩期峰值应变值在三组间差异均有统计学意义(P均<0.05)。结论早期左室构型正常组和向心肥厚组HT患者左室中间段纵向收缩期峰值应变值减低;左室后间隔基底段和中间段的纵向、径向、圆周向及三维应变值均减低;3D-STI可以检测到早期左室构型正常和向心肥厚HT患者左室局部心肌收缩功能的改变。 Objective To assess the left ventricular(LV)regional systolic function by three-dimensional speckle tracking imaging(3D-STI)echocardiography in early stage of different patterns of the left ventricle of heart in hypertensive patients.Methods 30 cases of hypertensive patients with normal geometric LV(normal geometric LVgroup),30 cases of concentric hypertrophy LV(concentric hypertrophy LV group)and 30 cases of normal subjects(control group)were selected as participants.LV longitudinal peak systolic strain,radial peak systolic strain,circumferential peak systolic strain,3-dimentional peak systolic strain were measured in all subjects by 3D-STI from the apical full-volume image and compared between groups.Results Compared with controls,systolic blood pressure(SBP),diastolic blood pressure(DBP),left atrium diameter(LAD),ascending aorta diameter(AO),left ventricular posterior wall thickness(LVPWT),left ventricular mass index(LVMI)increased significantly and E/A reduced significantly(P<0.01)in hypertensive patients with normal geometric LV and concentric hypertrophy LV.Compared with normal geometric LV,left atrium diameter(LAD),interventricular septum thickness(IVST),LVPWT,LVMI,relative wall thickness(RWT)increased significantly and E/A reduced significantly(P<0.01)in hypertensive patients with concentric hypertrophy LV.Compared with controls,the statistical differences were concentrated in the middle LV segments of the longitudinal peak systolic strain in patients with normal geometric LV and concentric hypertrophy LV.The LV peak systolic strain of longitudinal,radial,circumferential and 3-dimentional were significant differences in the basic and middle segments of LV posterior septum among the three groups(P<0.05).Conclusion The longitudinal peak systolic strain of the middle LV segments reduced in early hypertrophy patients with normal geometric LV and concentric hypertrophy LV.The LV peak systolic strain of longitudinal,radial,circumferential and 3-dimentional reduced in the basic and middle segments of LV posterio
