摘要
目的:分析经皮肾镜下气压弹道碎石联合超声碎石术处理复杂性肾结石的疗效。方法:选取2014年6月-2017年3月本院收治的复杂性肾结石患者50例,根据治疗方法不同将其分为观察组30例和对照组20例。观察组采用经皮肾镜下气压弹道碎石联合超声碎石术治疗,对照组采用开放手术治疗。比较两组患者的手术时间、治疗后1周和3个月的血清肌酐(serum creatinine,Scr)水平、并发症及结石的清除情况。结果:观察组手术时间为(62.22±10.64)min,显著短于对照组的(92.31±20.01)min(P<0.05);治疗后1周及3个月,观察组Scr水平均显著低于对照组(P<0.05);观察组并发症发生率为3.33%,显著低于对照组的30.00%(P<0.05);观察组结石清除率为93.33%,高于对照组的80.00%,但比较差异无统计学意义(P>0.05)。结论:给予复杂性肾结石患者经皮肾镜下气压弹道碎石联合超声碎石术,能有效缩短手术时间,促进肾功能恢复,减少并发症,提升结石清除率,值得推广。
Objective:To analyze the effects of pneumatic lithotripsy under percutaneous nephrolithotomy combined with ultrasonic lithotripsy in treatment of complicated renal stone.Method:A total of 50 patients with complicated renal stone in our hospital from June 2014 to March 2017 were selected.According to different treatment methods,they were divided into 30 cases of observation group and 20 cases of control group.The observation group was treated with pneumatic lithotripsy under percutaneous nephrolithotomy combined with ultrasonic lithotripsy,and the control group was treated with open surgery.The operation time,the levels of serum creatinine(Scr)after treatment 1 week and 3 months,complications and stone clearance rate were compared between the two groups.Result:The operative time of observation group was(62.22±10.64)min,which was significantly shorter than(92.31±20.01)min in control group(P<0.05).After treatment 1 week and 3 months,the levels of Scr in observation group were significantly lower than those of control group(P<0.05).The incidence of complications in the observation group was 3.33%,which was significantly lower than 30.00%of the control group(P<0.05).The stone clearance rate in the observation group was 93.33%,which was higher than 80.00%of the control group,but the difference was not statistically significant(P>0.05).Conclusion:The pneumatic lithotripsy under percutaneous nephrolithotomy combined with ultrasonic lithotripsy for patients with complicated renal stone,which can effectively shorten operation time,promote renal function recovery,reduce complications and improve stone clearance rate,thus it is worth promoting.
