摘要
目的分析脱落细胞联合组织多肽特异性抗原(TPS)、CA125检测在早期口腔癌诊断中的应用价值。方法选择2012年10月至2017年10月,本院收治的经病理确诊的口腔癌和癌前病变共79例,其中口腔癌患者39例,癌前病变40例,同时期健康志愿者40例。所有研究对象,均行脱落细胞学检查、血清TPS、CA125含量检测。结果早期口腔癌组TPS及CA125水平显著高于癌前病变组和对照组(P<0.05);同时早期口腔癌组TPS及CA125阳性率也显著高于癌前病变组(P<0.05)。脱落细胞DNA定量结果显示,早期口腔癌组的阳性率明显高于癌前病变组(χ~2=53.766,P<0.05)。脱落细胞联合TPS、CA125含量检测的检出率及阳性预测值均显著高于TPS、CA125含量(χ~2=5.556,8.919;P<0.05),略高于脱落细胞检测(P>0.05)。结论脱落细胞联合TPS、CA125检测可有效提高早期口腔癌的检出率,利于口腔癌患者的预防及治疗。
Objective To analyze the diagnostic value of the detection of exfoliative cells combined with TPS and CA125 for early oral cancer.Methods A total of 78 patients with oral cancer and precancerous lesions treated during Oct.2012 and Oct.2017 were involved,including 39 cases of oral cancer and 40 cases of precancerous lesions.Another 40 healthy controls were selected.The exfoliative cytology,serum TPS and CA125 were detected.Results The levels of TPS and CA125 in the cancer group were significantly higher than those in the precancerous group and control group(P<0.05).The positive rates of TPS and CA125 in the cancer group were significantly higher than those in the precancerous group(P<0.05).Exfoliative cell DNA quantitative results showed that the positive rate of cancer group was significantly higher than that of the precancerous group(χ2=53.766,P<0.05).The detection rate and positive predictive value of exfoliative cells combined with TPS and CA25 were significantly higher than those of TPS and CA25(χ2=5.556,8.919,P<0.05),and slightly higher than those of exfoliative cells(P>0.05).Conclusion The detection of exfoliative cells combined with TPS and CA125 contents can effectively improve the detection rate of early oral cancer,which is beneficial to the prevention and treatment of oral cancer.
