摘要
目的探讨布地奈德联合沙丁胺醇雾化吸入治疗小儿哮喘急性发作期的临床疗效及不良反应。方法将160例哮喘急性发作期患儿按入院的先后顺序分为A组和B组,每组80例。2组均给予常规治疗。在此基础上,A组给予吸入用布地奈德混悬液联合吸入用硫酸沙丁胺醇溶液雾化吸入治疗,B组给予吸入用复方异丙托溴铵溶液联合吸入用硫酸沙丁胺醇溶液雾化吸入治疗。观察2组治疗前、治疗7d后肺功能[第一秒用力呼气容积(FEV1)、用力肺活量(FVC)、呼气峰流量(PEF)]及血清免疫球蛋白E(IgE)水平的变化,咳嗽、气急、肺部干湿啰音的消失时间、住院费用和不良反应(心率加快、恶心呕吐、烦躁不安)发生率。结果与B组比较,A组治疗7d后FEV1、FVC、PEF均明显升高,血清IgE水平、住院费用均明显降低,A组咳嗽、气急、肺部干湿啰音的消失时间均缩短(均P<0.05)。A组总不良反应发生率与B组比较差异无统计学意义(6.25%比7.50%,P>0.05)。结论布地奈德联合沙丁胺醇治疗小儿哮喘急性发作期,可明显改善患儿的临床症状,提高患儿的肺功能,治疗效果较为理想,且安全可靠。
Objective To investigate the clinical efficacy and adverse effect of budesonide combined with salbutamol atomization inhalation in children with acute attack of asthma.Methods A total of 160 children received routine treatment for acute attack of asthma.According to the order of admission,these children were assigned to receive salbutamol sulfate atomization inhalation combined with either budesonide inhalation suspension(group A,n=80)or compound ipratropium bromide solution(group B,n=80).The forced expiratory volume in one second(FEV 1),forced vital capacity(FVC),peak expiratory flow(PEF),serum immunoglobulin E(IgE)level,disappearance time of cough,breath shortness and pulmonary rales,hospitalization costs,and adverse reactions(heart rate acceleration,nausea,vomiting,restlessness)were observed before and after treatment for 7 days.Results Compared with group B,the FEV 1,FVC and PEF were increased,serum IgE level and hospital costs were reduced,and disappearance time of cough,breath shortness and pulmonary rales were shortened in group A(P<0.05).The incidence of total adverse reactions in group A was not significantly different from that in group B(6.25%vs 7.50%,P>0.05).Conclusion Budesonide combined salbutamol atomization inhalation is an ideal,safe and reliable treatment that can obviously improve clinical symptoms and lung function in children with acute attack of asthma.
