摘要
目的研究并比较髓内钉与钢板内固定治疗胫骨远端关节外骨折的临床疗效。方法选取我院治疗的40例胫骨远端关节外骨折患者为研究对象,采取计算机单盲分组法分为A组和B组,每组20例。A组接受髓内钉内固定治疗,B组接受钢板内固定治疗。比较两组患者的治疗效果、手术及术后情况。结果 A组患者的术中出血量明显低于B组(P<0.05),但两组患者的手术时间、住院时间、骨折愈合时间、治疗优良率比较,差异无统计学意义(P>0.05);手术后,两组患者的足踝功能评分均明显增高(P<0.05),但组间比较,差异无统计学意义(P>0.05);A组患者的并发症发生率略高于B组,差异无统计学意义(P>0.05)。结论采用髓内钉和钢板对胫骨远端关节外骨折进行内固定治疗均具有显著的临床疗效,可有效促进足踝功能恢复,但髓内钉内固定术治疗后易发生并发症,需依照患者情况选用。
Objective To study and compare the clinical effect of intramedullary nail and plate internal fixation in the treatment of extra-articular distal tibial fractures.Methods A total of 40 cases of patients with extra-articular distal tibial fractures were selected in the study and divided into group A and group B by computer single blind grouping method,with 20 cases in each group.The group A was treated with intramedullary nail fixation,and the group B received plate internal fixation.The treatment effects,operation and postoperative situation were compared between the two groups.Results The amount of bleeding in the group A was significantly lower than that in the group B(P<0.05);there were no differences in the operation time,hospitalization time,fracture healing time and the excellent rate between the two groups(P>0.05).After the surgery,the ankle function scores in the two groups were significantly increased(P<0.05),but there was no statistical difference between the two groups(P>0.05).The incidence of complications in the group A was slightly higher than that in the group B,but the difference was not statistically significant(P>0.05).Conclusion Intramedullary nail and plate internal fixation in the treatment of extra-articular distal tibial fractures both have significant clinical efficacy and can effectively promote the functional recovery of ankle,but intramedullary nail internal fixation is prone to complications,the treatment method should be chosen according to the patient situation.
