摘要
目的探讨冰冻切片技术诊断乳腺癌肿瘤组织分级准确度及其影响因素。方法选取2015年3月至2017年5月本院收治的125例乳腺癌患者作为研究对象,采用回顾性分析法分析所有患者的临床资料,比较分析冰冻切片技术和术后病理在乳腺癌肿瘤组织分级的符合率,同时采用多因素Logistic回归分析影响冰冻切片技术诊断乳腺癌肿瘤组织分级准确度的相关影响因素。结果 (1)本研究125例乳腺癌患者术中冰冻切片诊断结果与术后病理诊断完全相符合者94例,符合率为75.20%;(2)经单因素分析发现:绝经前后、X线钼靶钙化点及肿物大小等临床因素具有统计学意义(P<0.05);(3)经非条件多因素Logistic回归模型分析发现:绝经后出现乳腺肿块、X线钼靶示多发钙化点减少及肿物组织学过大是影响术中冰冻切片诊断准确度的主要临床因素(P<0.05)。结论术中冰冻切片技术诊断乳腺癌肿瘤组织分级不能完全符合术后病理结果,绝经前后、X线钼靶示多发钙化点及肿物大小是影响术中冰冻切片技术诊断乳腺癌肿瘤组织分级准确度的主要临床因素。
Objective To investigate the accuracy and influencing factors of the frozen section technique in the diagnosis of breast cancer tumor grade.Methods A total of 125 breast cancer patients treated in our hospital between March 2015 and May 2017 were selected as subjects.Their clinical data was analyzed retrospectively.The coincidence rates of frozen sections and postoperative pathology in breast cancer were compared.At the same time,multivariate logistic regression analysis was used to analyze factors that influenced the accuracy of grade in the diagnosis of breast cancer tissue by frozen section technique.Results(1)Among the 125 cases of breast cancer patients,the frozen section diagnosis of 94 cases was consistent with postoperative pathological diagnosis,so the coincidence rate was 75.20%.(2)The results of univariate analysis showed that such clinical factors as calcification and size of mammography were statistically significant before and after menopause(P<0.05).(3)The results of non-conditional multivariate logistic regression analysis showed that post-menopausal breast lesions,the reduction of multiple calcification spots in X-ray molybdenum and oversized tumor tissue were the main clinical factors that affected the accuracy of intraoperative frozen section diagnosis(P<0.05).Conclusion The use of intraoperative frozen sections in the diagnosis of breast cancer tumor grade cannot fully conform to the postoperative pathological results.Before and after menopause,X-ray molybdenum target multiple calcification and tumor size are the main clinical factors that affect the accuracy of intraoperative frozen section technology in the diagnosis of breast cancer tumor grade.
